【摘要】：目的探討白血病患者外周中心靜脈導(dǎo)管(PICC)相關(guān)性血流感染的危險(xiǎn)因素。方法選擇2015年6月-2016年12月醫(yī)院收治的白血病PICC置管患者150例,統(tǒng)計(jì)PICC相關(guān)性血流感染患者臨床資料,經(jīng)單因素及Logistic多因素分析,探討PICC置管后發(fā)生相關(guān)性血流感染的危險(xiǎn)因素。結(jié)果 150例患者中出現(xiàn)導(dǎo)管相關(guān)性血流感染20例,感染率13.33%,共檢出病原菌20株,其中革蘭陰性菌10株占50.00%,以肺炎克雷伯菌和大腸埃希菌為主;革蘭陽性菌10株占50.00%,以表皮葡萄球菌為主;多因素logistic回歸分析結(jié)果顯示,年齡≥65歲、外周血象2.0×10~9/L、穿刺次數(shù)≥6次、化療次數(shù)≥11次、留置時(shí)間≥11個(gè)月、伴有糖尿病是PICC導(dǎo)管相關(guān)性血流感染的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論導(dǎo)致白血病患者PICC相關(guān)性血流感染的原因較多,應(yīng)針對危險(xiǎn)因素,化療前應(yīng)積極處理合并癥,選擇合理的導(dǎo)管種類、穿刺和封管方式,根據(jù)藥敏試驗(yàn)科學(xué)使用抗菌藥物,縮短置管時(shí)間,從而減少PICC相關(guān)性血流感染風(fēng)險(xiǎn)。
[Abstract]:Objective to investigate the risk factors of peripheral central venous catheter (PICC) associated blood flow infection in patients with leukemia. Methods from June 2015 to December 2016, 150 patients with leukemia PICC catheterization were selected. The clinical data of patients with PICC associated blood flow infection were analyzed by univariate and Logistic multivariate analysis. To investigate the risk factors of associated blood flow infection after PICC catheterization. Results among 150 patients, 20 cases had catheter-related blood flow infection, the infection rate was 13.33%.
