【摘要】：目的探討CEUS引導(dǎo)下穿刺活檢對(duì)前縱隔淋巴瘤的診斷價(jià)值。方法回顧性分析經(jīng)CEUS和常規(guī)超聲引導(dǎo)穿刺的各36例前縱隔淋巴瘤患者資料。比較CEUS引導(dǎo)組和常規(guī)超聲引導(dǎo)組的取材成功率和并發(fā)癥發(fā)生率。結(jié)果CEUS引導(dǎo)下前縱隔腫塊取材成功率為100%(36/36),其中非霍奇金淋巴瘤(NHL)26例,霍奇金淋巴瘤(HL)10例;36例患者,共穿刺取材60例次,并發(fā)癥發(fā)生率為11.11%(4/36)。常規(guī)超聲引導(dǎo)下前縱隔淋巴瘤取材成功率為88.89%(32/36),其中NHL 22例,HL 14例,共穿刺取材91例次,并發(fā)癥發(fā)生率為41.67%(15/36)。2組取材成功率和并發(fā)癥發(fā)生率差異均有統(tǒng)計(jì)學(xué)意義(χ2=4.235、8.651,P=0.040、0.003)。結(jié)論 CEUS可反映縱隔淋巴瘤的內(nèi)部的微循環(huán)情況,更好地指導(dǎo)靶向穿刺活檢,提高穿刺取材的成功率,減少并發(fā)癥。
[Abstract]:Objective to evaluate the diagnostic value of CEUS-guided biopsy in anterior mediastinal lymphoma. Methods the data of 36 patients with anterior mediastinal lymphoma treated by CEUS and conventional ultrasound guided puncture were retrospectively analyzed. The success rate and complication rate of CEUS guided group and conventional ultrasound guided group were compared. Results the success rate of anterior mediastinal mass extraction under CEUS guidance was 100% (36 / 36), including 26 cases of non-Hodgkin 's lymphoma (NHL) and 10 cases of Hodgkin's lymphoma (HL). Among the 36 patients, 60 were obtained by puncture, and the incidence of complications was 11.11% (4 / 36). The success rate of conventional ultrasound guided anterior mediastinal lymphoma was 88.89% (32 / 36), including 22 cases of NHL and 14 cases of HL. The incidence of complications was 41.67% (15 / 36). There were significant differences in the rate of success and the incidence of complications between the two groups (蠂 ~ 2 = 4.235 ~ 8.651P ~ (0.0400.003). Conclusion CEUS can reflect the internal microcirculation of mediastinal lymphoma, guide targeted biopsy better, improve the success rate of puncture and reduce complications.
