【摘要】：目的:評(píng)價(jià)血小板反應(yīng)蛋白-1(TSP-1)水平與維持性血液透析(MHD)患者微炎癥狀態(tài)間的相關(guān)性,探討其對(duì)MHD患者預(yù)后判斷的臨床價(jià)值。方法:選擇長期規(guī)律透析的MHD患者,隨訪觀察2年,定期檢測患者血中TSP-1水平和臨床及炎癥相關(guān)指標(biāo)水平。根據(jù)TSP-1水平不同,分為三組,比較組間上述檢測指標(biāo)的差異,采用Pearson簡單相關(guān)分析法分析TSP-1水平與炎癥相關(guān)指標(biāo)的相關(guān)性。同時(shí)觀察2年間入選患者的預(yù)后轉(zhuǎn)歸狀態(tài),利用Kaplan Meier生存曲線及對(duì)秩數(shù)檢驗(yàn)測定生存率,采用Cox回歸分析計(jì)算風(fēng)險(xiǎn)指數(shù)HR值及95%可信區(qū)間CI值。結(jié)果:TSP-1高水平組患者血中血脂及反應(yīng)體內(nèi)炎癥狀態(tài)相關(guān)指標(biāo)均高于TSP-1低水平組(P0.05),TSP-1水平和血脂及炎癥指標(biāo)間呈正相關(guān)。生存曲線分析結(jié)果顯示TSP-1高水平組及中等水平組患者的死亡率高于TSP-1低水平組。校正年齡、血脂、白蛋白及血白蛋白等因素后,進(jìn)一步行Cox風(fēng)險(xiǎn)模型分析結(jié)果示,TSP-1高水平的患者HR明顯高于TSP-1低水平組(P0.001),表明體內(nèi)高水平表達(dá)的TSP-1提示預(yù)后不良。結(jié)論:血清TSP-1水平與MHD患者微炎癥狀態(tài)密切相關(guān),且能作為判斷MHD患者預(yù)后的敏感指標(biāo)。
[Abstract]:Objective: to evaluate the correlation between platelet reactive protein-1 (TSP-1) level and microinflammation in (MHD) patients with maintenance hemodialysis (HD), and to evaluate its clinical value in judging the prognosis of MHD patients. Methods: MHD patients with long term regular dialysis were followed up for 2 years. The levels of serum TSP-1, clinical and inflammatory indexes were measured regularly. According to the different levels of TSP-1, three groups were divided into three groups. The correlation between TSP-1 level and inflammation related indexes was analyzed by Pearson simple correlation analysis. The survival rate was measured by Kaplan Meier survival curve and logarithmic number test. The HR value of risk index and 95% confidence interval CI value were calculated by Cox regression analysis. Results: the serum lipids and inflammatory status in the patients with high level of TSP-1 were significantly higher than those in the low level of TSP-1 (P0.05). There was a positive correlation between the level of TSP-1 and serum lipids and inflammatory indexes. Survival curve analysis showed that the mortality of patients with high and medium levels of TSP-1 was higher than that of low levels of TSP-1. After adjusting for age, blood lipid, albumin and albumin, the results of Cox risk model analysis showed that HR in patients with high TSP-1 level was significantly higher than that in low TSP-1 group (P0.001). High expression of TSP-1 suggests poor prognosis. Conclusion: serum TSP-1 level is closely related to microinflammation in MHD patients and can be used as a sensitive index to judge the prognosis of MHD patients.
