血栓彈力圖用于指導大量輸血患者血漿輸注的臨床觀察

【摘要】:目的:利用血栓彈力圖(Thrombelastograph,TEG)監(jiān)測大量輸血(massive transfusion,MT)患者凝血功能動態(tài)變化,以此為依據指導大量輸血患者新鮮冰凍血漿(fresh frozen plasma,FFP)的輸注,探討TEG在大量輸血患者凝血功能監(jiān)測中的作用及指導個體化精準血漿輸注方案。方法:選擇2015年5月~2016年12月術中進行大量輸血的病人90例,ASA分級Ⅱ~Ⅳ級,年齡20~52歲,體重52~83kg;颊叻譃槿M:大量血漿輸注組(D組)30例,RBC:FFP=1:1;少量血漿輸注組(S組)20例,RBC:FFP=2:1;TEG指導血漿輸注組(T組),40例。(注:1URBC:100ml FFP=1:1)記錄所有病人出血量、紅細胞(Red blood cell,RBC)與FFP的輸注量、晶膠體的輸注量及輸注血制品完成后即刻血栓彈力圖的參數(R、K、α和MA)。利用SPSS22.0統(tǒng)計軟件進行數據分析,p0.05認為差異有統(tǒng)計學意義。結果:三組患者的年齡、性別、身高、體重和ASA分級差異無統(tǒng)計學意義(p0.05)。三組患者在術中的出血量,RBC、晶體液和膠體液的輸注量,差異無統(tǒng)計學意義,(p0.05)。術中新鮮冰凍血漿的輸注量,D組明顯大于T組與S組,差異有統(tǒng)計學意義(p0.05),T組與S組比,T組大于S組,差異有統(tǒng)計學意義(p0.05)。TEG監(jiān)測的相關參數,與S組比,D組、T組的R值、K值明顯減小,α值和MA值明顯增大,差異有統(tǒng)計學意義(p0.05);T組與D組之間差異無統(tǒng)計學意義(p0.05)。結論:1、從血漿輸注量比較:TEG指導血漿輸注組明顯小于大量血漿輸注組,可避免不必要的血液制品浪費。2、從凝血功能方面比較,TEG指導血漿輸注組與大量血漿輸注組優(yōu)于少量血漿輸注組,能維持凝血功能狀態(tài)相對穩(wěn)定。3、TEG指導血漿輸注組中的紅細胞與血漿的輸注比例集中于(1~2):1。4、TEG適合用于大量輸血患者血漿輸注的指導,并能提供個體化的血漿輸注方案。
