【摘要】：目的探討三級(jí)醫(yī)院家庭護(hù)士服務(wù)模式對(duì)慢性病延續(xù)醫(yī)療服務(wù)的作用與影響。方法三級(jí)醫(yī)院成立家庭護(hù)士服務(wù)部及顧問(wèn)醫(yī)生團(tuán)隊(duì),與出院慢性病患者簽定延續(xù)醫(yī)療服務(wù)約定,并借助信息化平臺(tái),建立以簽約對(duì)象為中心,以家庭、社區(qū)、醫(yī)院為一體的無(wú)縫醫(yī)療服務(wù)鏈。統(tǒng)計(jì)2016年1—9月家庭護(hù)士服務(wù)模式應(yīng)用的相關(guān)指標(biāo)。結(jié)果家庭護(hù)士服務(wù)模式應(yīng)用后,26家社區(qū)衛(wèi)生服務(wù)中心與三級(jí)醫(yī)院簽約并成立醫(yī)療聯(lián)合體,262例慢性病出院患者與家庭護(hù)士服務(wù)部簽定延續(xù)醫(yī)療服務(wù)約定;慢性病簽約患者及社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)人員的滿意度分別為99.8%和90.0%等。結(jié)論家庭護(hù)士服務(wù)模式構(gòu)建與應(yīng)用,引導(dǎo)了慢性病患者科學(xué)合理就醫(yī)秩序,增強(qiáng)了三級(jí)醫(yī)院與社區(qū)衛(wèi)生機(jī)構(gòu)的上下聯(lián)動(dòng),提高了慢性病患者滿意度。
[Abstract]:Objective to explore the effect and influence of the service mode of family nurses in three-level hospitals on the continuous medical service of chronic diseases. Methods A family nurse service department and a team of consultant doctors were set up in the third level hospital to sign a continuous medical service agreement with the discharged patients with chronic diseases. With the help of the information platform, the hospital set up a contract centered on the target, taking the family and the community as the center. Hospital as one of the seamless medical service chain. Statistics on the application of family nurse service model in January to September 2016. Results after the application of the family nurse service model, 26 community health service centers signed with the tertiary hospitals and set up a medical association. 262 patients with chronic diseases and the family nurse service department signed a continuation of the medical service agreement. The satisfaction of chronic patients and community health service personnel were 99.8% and 90.0% respectively. Conclusion the construction and application of family nurse service model can guide the patients with chronic diseases to seek medical treatment scientifically and reasonably, strengthen the linkage between tertiary hospitals and community health institutions, and improve the satisfaction of patients with chronic diseases.
