基于�？谱o(hù)士核心能力的兵團(tuán)手術(shù)室專科護(hù)士培訓(xùn)方案研究

【摘要】：目的:以手術(shù)室�？谱o(hù)士核心能力培養(yǎng)為目標(biāo)導(dǎo)向,構(gòu)建兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)方案,包括學(xué)員入選標(biāo)準(zhǔn)、師資要求、培養(yǎng)時(shí)間、方式、培訓(xùn)目標(biāo)、培訓(xùn)考核、培訓(xùn)內(nèi)容等。并運(yùn)用德爾菲專家咨詢法進(jìn)行評(píng)價(jià),最終確定一套標(biāo)準(zhǔn)、統(tǒng)一的兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)方案,可為手術(shù)室�？谱o(hù)士的培養(yǎng)提供參考依據(jù),使培訓(xùn)工作系統(tǒng)化,提高培訓(xùn)質(zhì)量和效果。方法:1.文獻(xiàn)分析法:通過查閱文獻(xiàn),確定手術(shù)室�？谱o(hù)士核心能力的框架,包括專業(yè)品質(zhì)、圍手術(shù)期護(hù)理能力、評(píng)判性思維能力、管理能力、交流溝通能力、專業(yè)發(fā)展能力,六個(gè)核心能力。2.專家咨詢法:基于兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)現(xiàn)況,以�？谱o(hù)士核心能力為目標(biāo)指引,擬定培訓(xùn)方案,并運(yùn)用兩輪德爾菲法進(jìn)行評(píng)價(jià),通過專家咨詢的積極系數(shù)、權(quán)威系數(shù)、變異系數(shù)、協(xié)調(diào)系數(shù)等指標(biāo),對(duì)咨詢結(jié)果進(jìn)行可靠性和科學(xué)性的檢驗(yàn),最終確定兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)方案。結(jié)果:1.函詢專家:第一輪36名,第二輪35名,包括臨床護(hù)理專家、護(hù)理管理專家、護(hù)理教育專家和臨床外科手術(shù)醫(yī)生,均來(lái)自西北地區(qū)(陜西、甘肅、青海、寧夏、新疆)手術(shù)室�？谱o(hù)士培訓(xùn)基地的三級(jí)綜合醫(yī)院,具有較高的學(xué)科代表性和地區(qū)代表性。2.兩輪專家咨詢結(jié)果可靠性:兩輪專家咨詢的積極系數(shù)分別為100%、97.2%,表明專家的積極性高;專家權(quán)威程度系數(shù)Cr為0.901和0.908,可信度較高,具有較好的學(xué)科代表性;第二輪培訓(xùn)方案專家協(xié)調(diào)系數(shù)為0.505(P0.00),理論課程專家協(xié)調(diào)系數(shù)0.533(P0.00),實(shí)踐課程專家協(xié)調(diào)系數(shù)為0.392(P0.00),顯著性檢驗(yàn)具有統(tǒng)計(jì)學(xué)意義,表明專家意見協(xié)調(diào)程度較好,咨詢結(jié)果的可靠性強(qiáng)。3.確定了兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)方案,包括學(xué)員入選標(biāo)準(zhǔn)、師資要求、培養(yǎng)時(shí)間、方式、培訓(xùn)目標(biāo)、培訓(xùn)考核、培訓(xùn)內(nèi)容等。共49個(gè)條目,重要性性評(píng)分均值范圍在4.30~5.00之間,標(biāo)準(zhǔn)差為0.000~0.531,變異系數(shù)為0.000~0.136。4.確定了具體的培訓(xùn)內(nèi)容:理論課程和實(shí)踐課程:理論課程共70個(gè)內(nèi)容,重要性性評(píng)分均值范圍在4.06~4.97之間,標(biāo)準(zhǔn)差為0.148~0.613,變異系數(shù)為0.029~0.145,實(shí)踐課程38個(gè)內(nèi)容,重要性性評(píng)分均值范圍在4.14~4.96之間,標(biāo)準(zhǔn)差為0.209~0.560,變異系數(shù)為0.042~0.135。結(jié)論:本研究構(gòu)建的兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)方案具有可靠性和科學(xué)性,可為兵團(tuán)手術(shù)室�？谱o(hù)士培訓(xùn)提供參考依據(jù)。
[Abstract]:Objective: to set up a training program for operating room nurses, including the selection criteria, teachers' requirements, training time, methods, training objectives, training assessment, training contents and so on, based on the training of operating room nurses' core competence. Using Delphi expert consultation method to evaluate and finally determine a set of standards, a unified training program for specialized nurses in the operating room of the corps, which can provide a reference basis for the training of specialized nurses in operating rooms, and systematize the training work. Improve the quality and effect of training. Method 1: 1. Literature analysis: through literature review, the framework of core competence of operating room nurses was determined, including professional quality, perioperative nursing ability, critical thinking ability, management ability, communication ability, professional development ability, etc. Six core competencies. The method of expert consultation: based on the present situation of specialist nurse training in the operating room of the Corps, taking the core competence of the specialist nurse as the target guide, the training program is drawn up and evaluated by using two rounds of Delphi method. The positive coefficient and authority coefficient of expert consultation are adopted. Variation coefficient, coordination coefficient and other indexes, the reliability and scientific test of consultation results were carried out, and the training scheme of specialized nurses in operating room of the corps was finally determined. The result is 1: 1. Letter experts: 36 in the first round, 35 in the second, including clinical nursing experts, nursing management experts, nursing education experts and clinical surgeons, all from the Northwest Territories (Shaanxi, Gansu, Qinghai, Ningxia, Xinjiang) operating room specialized nurse training base level 3 general hospital, has the higher discipline representativeness and the district representative. The reliability of the results of two rounds of expert consultation: the positive coefficient of the two rounds of expert consultation is 100 ~ 97.22, which indicates that the enthusiasm of the expert is high, the expert authority coefficient Cr is 0.901 and 0.908, the credibility is high, and the expert is representative of the subject. The coordination coefficient of experts in the second round of training programs is 0.505 (P0.00), the coordination coefficient of experts in theoretical courses is 0.533 (P0.00), and the coordination coefficient of experts in practical courses is 0.392 (P0.00). The significance test shows that the coordination degree of expert opinions is good and the reliability of consultation results is strong. 3. The training scheme of specialized nurses in the operating room of the corps was determined, including the selection standard, the requirement of teachers, the training time, the way, the training target, the training examination, the training content and so on. A total of 49 items, the mean range of importance score was between 4.30 and 5.00, the standard deviation was 0.000 ~ 0.531, and the coefficient of variation was 0.000 ~ 0.136.4. The specific training contents were determined: theory course and practice course: there were 70 contents in theory course, the mean value of importance score was between 4.06 and 4.97, the standard deviation was 0.1480.6313, the coefficient of variation was 0.0290.145, and the content of practical course was 38. The mean range of importance score was 4.14 ~ 4.96, the standard deviation was 0.209 ~ 0.560, and the coefficient of variation was 0.042 ~ 0.135. Conclusion: the training scheme of special nurses in operating room is reliable and scientific, which can provide reference for the training of special nurses in operating room of corps.
