【摘要】：目的:探討甲狀旁腺切除對(duì)血液透析患者頸-股動(dòng)脈脈搏波速度(cfPWV)的影響。方法:選取規(guī)范藥物治療無(wú)效行甲狀旁腺全切(tPTX)的重度繼發(fā)性甲旁亢患者35例作為治療組,選取健康志愿者30例作為對(duì)照組。治療組術(shù)前與術(shù)后1年分別檢測(cè)肝腎功能、血常規(guī)、全段甲狀旁腺激素(iPTH)、cfPWV,對(duì)照組測(cè)量cfPWV。比較術(shù)前與術(shù)后1年患者臨床及實(shí)驗(yàn)室指標(biāo)、cfPWV變化;治療組1年后cfPWV與對(duì)照組進(jìn)行比較。結(jié)果:術(shù)后1年患者iPTH、血清鈣、血清磷較術(shù)前顯著下降,差異有統(tǒng)計(jì)學(xué)意義(P0.01);術(shù)后1年患者血紅蛋白、血漿白蛋白水平較術(shù)前顯著升高,差異有統(tǒng)計(jì)學(xué)意義(P0.01);術(shù)后1年患者cf PWV較術(shù)前顯著下降,差異有統(tǒng)計(jì)學(xué)意義(P0.01);治療組患者術(shù)后1年cf PWV顯著高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論:t PTX可降低血液透析患者cfPWV。
[Abstract]:Objective: to investigate the effect of parathyroidectomy on carotid artery pulse wave velocity (cfPWV) in hemodialysis patients. Methods: 35 patients with severe secondary hyperthyroidism who had no effect on parathyroid gland resection (tPTX) were selected as treatment group and 30 healthy volunteers as control group. The liver and kidney function, blood routine, whole parathyroid hormone (iPTH) and cfPWV were measured before and 1 year after operation in the treatment group and cfPWV. in the control group. The clinical and laboratory indexes were compared between preoperative and postoperative year, and the cfPWV of the treatment group was compared with that of the control group after 1 year. Results: the levels of serum calcium and phosphorus in iPTH, patients were significantly lower than those before operation (P0.01), the levels of hemoglobin and plasma albumin were significantly higher than those before operation (P0.01). The difference was statistically significant (P0.01); the cf PWV of the patients after 1 year was significantly lower than that of the pre-operation (P0.01); the cf PWV of the treatment group was significantly higher than that of the control group (P0.01) one year after operation, the difference was statistically significant (P0.01). Conclusion the cfPWV. of hemodialysis patients can be decreased by PTX.
