【摘要】：目的:觀察應(yīng)用彈力繃帶進(jìn)行膝踝關(guān)節(jié)聯(lián)合捆扎治療對(duì)改善腦卒中患者步態(tài)及日常生活活動(dòng)能力的臨床療效。方法:40例腦卒中患者隨機(jī)分為試驗(yàn)組和對(duì)照組,兩組患者均接受腦卒中后常規(guī)康復(fù)治療,試驗(yàn)組患者在常規(guī)康復(fù)治療中的站立、邁步、行走等訓(xùn)練時(shí)應(yīng)用彈力繃帶對(duì)患側(cè)膝、踝關(guān)節(jié)進(jìn)行同步捆扎,保持膝關(guān)節(jié)微屈、踝關(guān)節(jié)背屈。在治療前及治療8周后進(jìn)行三維步態(tài)分析、患側(cè)下肢Fugl-Meyer運(yùn)動(dòng)功能評(píng)定(FMA-L)及改良Barthel指數(shù)(MBI)評(píng)分,試驗(yàn)組患者在首次應(yīng)用彈力繃帶捆扎前后各做1次三維步態(tài)分析。結(jié)果:試驗(yàn)組患者在首次應(yīng)用彈力繃帶捆扎后三維步態(tài)分析顯示其患側(cè)步長(zhǎng)、步速及支撐相髖、膝、踝各關(guān)節(jié)力矩峰值各項(xiàng)分值較捆扎前有顯著改善(P0.05);治療8周后試驗(yàn)組患者下肢FMA-L、MBI評(píng)分及三維步態(tài)數(shù)據(jù)較對(duì)照組有顯著改善(P0.05)。結(jié)論:應(yīng)用彈力繃帶進(jìn)行膝踝關(guān)節(jié)聯(lián)合捆扎可顯著提高卒中后偏癱患者的異常步態(tài)及日常生活活動(dòng)能力。
[Abstract]:Objective: to observe the clinical effect of knee and ankle joint binding with elastic bandage on improving gait and ADL of stroke patients. Methods 40 stroke patients were randomly divided into two groups: the experimental group and the control group. The patients in both groups were treated with routine rehabilitation therapy after stroke. The patients in the test group were treated with elastic bandage on the contralateral knee during the training of standing, stepping and walking. The ankle joint carries on the synchronous banding, maintains the knee joint slightly flexion, the ankle joint back flexion. Three-dimensional gait analysis was performed before treatment and 8 weeks after treatment. Fugl-Meyer motor function assessment (FMA-L) and modified Barthel index (MBI) score of the affected lower extremities were evaluated. The patients in the experimental group underwent three dimensional gait analysis before and after the first application of elastic bandage. Results: the three dimensional gait analysis of the patients in the test group after the first application of elastic bandage showed that the affected side step, step speed and supporting phase of hip, knee, hip and knee. After 8 weeks of treatment, the FMA-LU MBI score and 3D gait data of the patients in the experimental group were significantly improved than those in the control group (P0.05). Conclusion: the application of elastic bandage in knee and ankle joint binding can significantly improve the abnormal gait and activity of daily living in patients with hemiplegia after stroke.
