【摘要】：目的探討穴位按壓下自由體位聯(lián)合適度保護會陰分娩法對分娩結(jié)局的影響。方法選取2014年6月—2016年6月海南省某婦幼保健院250例愿意自然分娩的孕產(chǎn)婦作為研究對象,用隨機數(shù)字表法分為實驗組125例和對照組125例。兩組均采用自由體位聯(lián)合適度保護會陰分娩法助產(chǎn)。在此基礎(chǔ)上,對實驗組進行穴位按壓,記錄并比較兩組產(chǎn)婦剖宮產(chǎn)率、會陰側(cè)切率、產(chǎn)鉗助產(chǎn)率、會陰裂傷、產(chǎn)時疼痛程度、產(chǎn)后出血率、新生兒窒息率、體重及阿普加評分。結(jié)果最終共231例產(chǎn)婦完成該研究,其中實驗組117例、對照組114例。實驗組與對照組比較,剖宮產(chǎn)率及會陰側(cè)切率降低、會陰裂傷及疼痛程度減輕、產(chǎn)后出血減少、新生兒阿普加評分提高,差異均具有統(tǒng)計學意義(P0.05或P0.01)。結(jié)論穴位按壓下自由體位聯(lián)合適度保護會陰分娩法在降低產(chǎn)婦剖宮產(chǎn)及會陰側(cè)切率、減少會陰裂傷、緩解產(chǎn)時疼痛、防止產(chǎn)后出血及預(yù)防新生兒窒息方面有一定效果,可改善產(chǎn)婦及新生兒的分娩結(jié)局。
[Abstract]:Objective to investigate the effect of free posture combined with moderate protection of perineum delivery method on delivery outcome under acupoint pressure. Methods from June 2014 to June 2016, 250 pregnant and parturient women who were willing to give birth naturally in a hospital of maternal and child health care in Hainan Province were selected as study objects. 125 cases were divided into experimental group (125 cases) and control group (125 cases) by random digital table. Both groups were given free posture combined with moderate protection of perineal delivery. On this basis, acupressure was performed to record and compare the cesarean section rate, perineum side incision rate, forceps delivery rate, perineal laceration, labor pain degree, postpartum hemorrhage rate, asphyxia rate of newborn, weight and Apgar score in the experimental group. Results 231 puerperas were enrolled in the study, 117 in the experimental group and 114 in the control group. Compared with the control group, the cesarean section rate and perineal incision rate decreased, perineal laceration and pain reduced, postpartum hemorrhage decreased, the Apgar score of newborn increased, the difference was statistically significant (P0.05 or P0.01). Conclusion the free position under acupoint pressure combined with moderate protection of perineum delivery has certain effect on reducing cesarean section and perineum side cutting rate, reducing perineal laceration, relieving pain during labor, preventing postpartum hemorrhage and preventing neonatal asphyxia. It can improve the delivery outcome of parturient and newborn.
