Extended Tofts與Exchange模型的動態(tài)對比增強MRI參數(shù)在鑒別細胞型子宮肌瘤中應(yīng)用

【摘要】:目的探討Extended Tofts和Exchange兩種模型的動態(tài)對比增強MRI(DCE-MRI)參數(shù)在鑒別細胞型子宮肌瘤中的應(yīng)用價值。方法搜集2015年10月至2016年6月經(jīng)手術(shù)病理證實的32例子宮肌瘤患者,術(shù)前行盆腔多期DCE-MRI,選擇Extended Tofts和Exchange血流動力學模型分別計算子宮肌瘤的定量灌注參數(shù)K~(trans)、PS、Fp、K_(ep)、V_e、V_p。術(shù)后按病理分型分為細胞型、普通型、退變型3組,對比分析3組子宮肌瘤灌注參數(shù)的組間差異;篩選出有統(tǒng)計學意義的參數(shù),繪制鑒別細胞型子宮肌瘤的ROC曲線;使用Pearson相關(guān)性分析觀察兩種模型間的參數(shù)有無相關(guān)性。結(jié)果在3組子宮肌瘤間,Extended Tofts模型中的K~(trans)、Kep、Vp及Exchange模型中的Fp、K_(ep)、V_p值差異有統(tǒng)計學意義(P0.05),且這些參數(shù)在鑒別細胞型子宮肌瘤的ROC曲線下面積分別為1.000、0.923、0.827、1.000、0.702、0.738;Extended Toft模型中的K~(trans)與Exchange模型中的PS和Fp相關(guān)(r=0.365,P=0.034,r=0.968,P=0.000),K_(ep)、V_p在兩種模型中呈正相關(guān)(r=0.454,P=0.007,r=0.479,P=0.004),兩種模型間的Ve值無相關(guān)性。結(jié)論 Extended Tofts模型中的K~(trans)、K_(ep)、V_p及Exchange模型中的Fp、K_(ep)、V_p值在鑒別細胞型子宮肌瘤中的應(yīng)用價值較高,特別是Extended Tofts模型中的K~(trans)值和Exchange模型中的Fp值診斷效能最高,且兩者具有明顯的相關(guān)性;同時Extended Tofts模型的K_(ep)、V_p值診斷效能優(yōu)于Exchange模型。
[Abstract]:Objective to explore the application value of dynamic contrast enhanced MRI (DCE-MRI) parameters of two models of Extended Tofts and Exchange in the identification of cell myoma of the uterus. Methods 32 cases of uterine myoma confirmed by surgical pathology from October 2015 to 2016 were collected and the pelvic multistage DCE-MRI was performed before operation, and Extended Tofts and Exchange hemodynamics were selected. The quantitative perfusion parameters K~ (trans), PS, Fp, K_ (EP), V_e, V_p. were separately calculated and divided into 3 groups of cells, common type and degeneration type, and compared and analyzed the differences between the 3 groups of uterine myoma perfusion parameters. The statistical parameters were selected to draw the ROC curve of the differentiation of the myoma of the uterus; the Pearson phase was used. There was no correlation between the parameters between the two models. Results in the 3 groups of uterine myomas, the K~ (trans), Kep, Vp and Exchange models were Fp, K_ (EP), V_p values in the Extended Tofts model. The K~ (trans) in the Extended Toft model is related to the PS and Fp in the Exchange model (r=0.365, P=0.034, r=0.968, P=0.000), and there is a positive correlation between the two models. The value of Fp, K_ (EP) and V_p is of high value in the identification of cell myoma of the uterus, especially the K~ (trans) value in the Extended Tofts model and the Fp value in the Exchange model is the highest, and the two have obvious correlation. Meanwhile, the Extended Tofts model is better than the model.
