【摘要】：目的:評(píng)估頭體一體式固定板在全中樞放射治療(CSI)中的應(yīng)用。方法:回顧性分析43例使用頭體一體式固定板進(jìn)行全中樞放射治療的患者,在治療過程中將掃描圖像與數(shù)字重建放射影像(DRR)進(jìn)行配準(zhǔn),其中包括頭頸段、胸段和腰骶段3部位圖像,自動(dòng)配準(zhǔn)調(diào)整患者在X軸、Y軸和Z軸方向的擺位誤差,并采用SPSS 22.0軟件進(jìn)行數(shù)據(jù)分析。結(jié)果:頭頸段、胸段和腰骶段X軸擺位誤差分別為(0.122±0.120)cm、(0.074±0.211)cm和(0.083±0.096)cm;Y軸擺位誤差分別為(0.202±0.154)cm、(0.236±0.175)cm和(0.283±0.187)cm;Z軸擺位誤差為(0.136±0.127)cm、(0.249±0.472)cm和(0.272±0.211)cm,3個(gè)方向均小于臨床設(shè)定誤差范圍(頭0.3 cm,體0.5 cm),差異具有統(tǒng)計(jì)學(xué)意義,頭頸段(t=-13.091,t=-5.598,t=-11.423;P0.05);胸段(t=-17.814,t=-13.325,t=-4.699;P0.05);腰骶段(t=-33.632,t=-8.974,t=-8.390;P0.05)。兒童與成人頭頸段、胸段和腰骶段3部位的X軸、Y軸和Z軸3個(gè)方向比較均無差異,頭頸段(t=0.698,t=1.028,t=1.691;P0.05);脊髓段(t=1.376,t=0.469,t=1.179;P0.05)。頭頸段、胸段和腰骶段兩兩比較中,X軸方向無差異(F=2.035,P0.05),Y軸、Z軸方向差異有統(tǒng)計(jì)學(xué)意義(F=3.777,F=3.895;P0.05)。結(jié)論:在CSI中應(yīng)用頭體一體式固定板新方法對(duì)患者進(jìn)行仰臥體位固定,其擺位誤差小,操作方便,患者順從度和舒適度明顯提高,兒童及成人均可使用。
[Abstract]:Objective: to evaluate the application of head-body integrated plate in CSI. Methods: a retrospective analysis was made of 43 patients who underwent total central radiotherapy with head and body fixation plates. During the course of treatment, scanning images were registered with digital reconstruction radiography (DRR), including head and neck segments. Chest segment and lumbosacral segment images of 3 parts were automatically registered to adjust the position errors in X axis Y axis and Z axis. SPSS 22.0 software was used to analyze the data. Results: the axial pendulum errors of head and neck segment, thoracic segment and lumbosacral segment were 0.122 鹵0.120 0.211)cm 鹵0.074 鹵0.211)cm and 0.083 鹵0.096 cm ~ (-1) respectively. The errors were 0.202 鹵0.154 cm ~ (-1) and 0.283 鹵0.187 cm ~ (Z), 0.136 鹵0.127 cm ~ (2) 0.472)cm and 0.272 鹵0.211 cm ~ (-1) respectively. The three directions were all smaller than the clinical error range (0. 3 cm, 0. 5 cm 路m ~ (-1), 0. 5 cm 路cm ~ (-1), the difference was statistically significant. In the head and neck segment, there was no significant difference between the two groups (P 0.05). The head and neck segment of the head and neck segment was composed of ttio -13.091t ~ 5.598t ~ (-1) -11.423 ~ (th) P 0.05, T ~ (17. 814) ~ 13.325 ~ t ~ (-4.699) T ~ (0.05), and the lumbosacral segment ~ (33. 632) t ~ (-8.974t) ~ 8.390m ~ (th) P 0.05 ~ (th). There was no difference in the X axis Y axis and Z axis between children and adults in the head and neck segment, thoracic segment and lumbosacral segment. There was no difference in the direction of X axis between head and neck segment, thoracic segment and lumbosacral segment. Conclusion: a new method for supine position fixation with head and body type fixation plate is used in CSI. It has the advantages of low error, convenient operation, higher degree of compliance and comfort, and can be used in children and adults.
