Mur血型抗原抗體調(diào)查及應(yīng)用研究

【摘要】:目的了解廣東地區(qū)人群抗-Mur發(fā)生頻率、性質(zhì)和來(lái)源以及Mur抗原的分布;建立獻(xiàn)血者M(jìn)ur稀有血型庫(kù)服務(wù)于臨床用血。方法以微量板的鹽水法和聚凝胺法對(duì)廣東省中山、深圳、江門(mén)、東莞、惠州市34 419名獻(xiàn)血者做抗-Mur篩查,用抗-Mur做紅細(xì)胞Mur抗原篩查,部分標(biāo)本采用固相免疫吸附試驗(yàn)(SPIA)篩查。結(jié)果本組篩查對(duì)象共檢出抗-Mur 48例,占0.14%(48/34 419),其中IgM占79.2%(38/48),IgM+IgG占16.7%(8/48),IgG占4.2%(2/48);獻(xiàn)血者抗-Mur頻率比較,無(wú)顯著性差異(P0.05);中山患者抗-Mur頻率為0.17%(5/3010),與獻(xiàn)血者比較無(wú)明顯差異(P0.05);中山地區(qū),Mur抗原在獻(xiàn)血者、患者中的發(fā)生率分別為7.31%(128/1 752)vs 7.14%(80/1 120),P0.05,獻(xiàn)血者男性和女性中分別為7.7%(72/931)vs 6.5%(33/504),P0.05;本組Mur陽(yáng)性者均為漢族,其中廣東省籍貫占66.7%(70/105);建立了中山地區(qū)獻(xiàn)血者M(jìn)ur稀有血型庫(kù);SPIA技術(shù)從2 541份血漿標(biāo)本中檢出4例抗-Mur。結(jié)論 SPIA可以檢出37℃活性有較高臨床意義的抗-Mur,廣東地區(qū)獻(xiàn)血人群Mur血型抗體、抗原發(fā)生頻率均明顯高于Rh D血型抗體、抗原發(fā)生頻率,不規(guī)則抗體檢測(cè)應(yīng)增加Mur陽(yáng)性篩選紅細(xì)胞,供受血者均應(yīng)檢測(cè)Mur血型是否相合,以進(jìn)一步提高輸血免疫安全性。
[Abstract]:Objective to investigate the frequency, nature and origin of anti--Mur and the distribution of Mur antigen in Guangdong population, and to establish Mur rare blood group bank for clinical use. Methods 34,419 blood donors from Zhongshan, Shenzhen, Jiangmen, Dongguan and Huizhou were screened for anti--Mur and anti--Mur for erythrocyte Mur antigen screening, and some samples were screened by solid phase immunosorbent assay (SPIA). Results A total of 48 cases of anti--Mur were detected in this group, accounting for 0.14% of 34 419 cases, in which IgM accounted for 79.2% / 48% IgM IgG accounted for 16.710% and 8 / 48% of IgG accounted for 4.2% of 48%, and the frequency of anti--Mur in blood donors was compared.
