【摘要】：目的利用彩色多普勒超聲對脛骨橫向骨搬移術(shù)治療老年糖尿病足(DF)的臨床應(yīng)用進(jìn)行評價。方法老年DF患者38例,行脛骨橫向骨搬移術(shù)治療,采用彩色多普勒超聲對手術(shù)前后老年患足的下肢動脈管壁、管腔及血流動力學(xué)改變進(jìn)行評價。結(jié)果經(jīng)脛骨橫向骨搬移術(shù)治療后,老年患者下肢動脈管徑彩超斑塊率、狹窄率、閉塞率均較手術(shù)前下降,但只有狹窄率手術(shù)前后的差異有統(tǒng)計學(xué)意義(P0.05);手術(shù)后,患者的兩側(cè)股總動脈、乆動脈、脛后動脈和足背動脈的管腔內(nèi)徑均明顯增大(均P0.05);兩側(cè)股總動脈、乆動脈、脛后動脈和足背動脈的血流平均速度均明顯減慢(均P0.05);兩側(cè)股總動脈、乆動脈、脛后動脈和足背動脈的血流量均明顯增加(均P0.05)。結(jié)論彩色多普勒超聲檢測能客觀反映脛骨橫向骨搬移術(shù)后老年DF患者的血管病變程度,無創(chuàng)傷、簡便易行、便于老年人接受,有助于對脛骨橫向骨搬移術(shù)的臨床效果提供血流動力學(xué)的依據(jù)。
[Abstract]:Objective to evaluate the application of color Doppler ultrasound in the treatment of senile diabetic foot disease (DFF). Methods 38 cases of senile DF were treated with transversal bone transfer of tibia. The changes of arterial wall, lumen and hemodynamics of lower extremity were evaluated by color Doppler ultrasound before and after operation. Results the plaque rate, stenosis rate and occlusion rate of lower extremity artery diameter in elderly patients were decreased after lateral tibial bone transfer, but only the difference before and after operation was statistically significant (P 0.05). The lumen diameters of the common femoral artery, the posterior tibial artery and the dorsal foot artery were significantly increased (all P 0.05). The mean blood flow velocity of posterior tibial artery and dorsalis pedis artery was significantly slower (all P 0.05), and the blood flow of bilateral common femoral artery, body and female artery, posterior tibial artery and dorsal foot artery were significantly increased (all P 0.05). Conclusion Color Doppler ultrasound can objectively reflect the degree of vascular lesion in elderly patients with DF after tibial lateral bone transplantation. It is simple and easy for the elderly to accept. It is helpful to provide hemodynamic basis for the clinical effect of lateral tibial bone transfer.
