超聲彈性成像對腱鞘巨細(xì)胞瘤的輔助診斷價值

【摘要】:目的探討超聲彈性成像技術(shù)對腱鞘巨細(xì)胞瘤的輔助診斷價值。方法選擇因手部腫物于我科檢查的102例患者。全部病例均經(jīng)病理證實。采用彈性成像技術(shù)測量彈性適配器與手部腫物的應(yīng)變比值(SR),應(yīng)用受試者工作特性曲線評價超聲彈性成像對腱鞘巨細(xì)胞瘤的診斷價值。結(jié)果 (1)傳統(tǒng)二維超聲診斷腱鞘巨細(xì)胞瘤的靈敏度、特異度、準(zhǔn)確性、陽性預(yù)測值及陰性預(yù)測值分別為82.2%、68.4%、74.5%、67.3%、83.0%。(2)應(yīng)用ROC曲線評價SR診斷腱鞘巨細(xì)胞瘤的曲線下面積為0.928,取SR診斷閾值為10.15。(3)利用常規(guī)二維超聲與超聲彈性成像進(jìn)行聯(lián)合診斷腱鞘巨細(xì)胞瘤時,診斷的靈敏度、特異度、準(zhǔn)確性、陽性預(yù)測值及陰性預(yù)測值分別為91.1%、93.0%、92.2%、91.1%、93.0%。結(jié)論超聲彈性參數(shù)SR對腱鞘巨細(xì)胞的診斷有較好的應(yīng)用價值,常規(guī)二維超聲與超聲彈性成像聯(lián)合診斷可顯著提高診斷效能。
[Abstract]:Objective to evaluate the value of ultrasonic elastography in the diagnosis of giant cell tumor of tendon sheath. Methods 102 patients with hand mass were examined in our department. All cases were confirmed by pathology. The strain ratio of elastic adaptor to hand mass was measured by elastic imaging technique. The diagnostic value of ultrasonic elastic imaging for giant cell tumor of tendon sheath was evaluated by using the operating characteristic curve of the subjects. Results 1) the sensitivity, specificity and accuracy of traditional two-dimensional ultrasonography in the diagnosis of giant cell tumor of tendon sheath. The positive predictive value and the negative predictive value were 82.2 and 74.5and 67.33.0, respectively.) the area under the curve of SR diagnosis of giant cell tumor of tendon sheath was 0.928 by ROC curve, and the diagnostic threshold of SR was 10.15. 3) when using conventional two-dimensional ultrasound and ultrasonic elastic imaging to diagnose giant cell tumor of tendon sheath, The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 91.1 and 93.0, respectively. Conclusion Ultrasonic elastic parameter SR has a good value in the diagnosis of giant cells of tendon sheath. The combined diagnosis of conventional two-dimensional ultrasound and ultrasonic elastic imaging can improve the diagnostic efficiency significantly.
