【摘要】：目的探討血漿中性粒細(xì)胞明膠蛋白酶相關(guān)脂質(zhì)運(yùn)載蛋白(NGAL)水平對(duì)嚴(yán)重膿毒癥患者死亡和多器官功能障礙綜合征(MODS)的預(yù)測(cè)價(jià)值。方法分析123例嚴(yán)重膿毒癥和膿毒性休克患者血漿NGAL水平與發(fā)生MODS和死亡的相關(guān)性。結(jié)果 7d內(nèi)47例發(fā)生MODS[第1天10例(8%),第7天37例(30%)]。12個(gè)月內(nèi)發(fā)生死亡39例(32%)。高血漿NGAL水平組較低水平組出現(xiàn)MODS和死亡的風(fēng)險(xiǎn)更高(P0.05)。ROC曲線分析顯示,血漿NGAL水平對(duì)ICU內(nèi)死亡病例有預(yù)測(cè)價(jià)值。Cox回歸分析在校正混雜的危險(xiǎn)因素后顯示,血漿NGAL水平升高是死亡(HR=2.128,95%CI=1.078~4.203,P0.05)和MODS(HR=1.896,95%CI=1.012~3.552,P0.05)的獨(dú)立預(yù)測(cè)因素。結(jié)論血漿NGAL水平升高是嚴(yán)重膿毒癥患者M(jìn)ODS和死亡的獨(dú)立預(yù)測(cè)因素。
[Abstract]:Objective to investigate the predictive value of plasma neutrophil gelatinase-associated lipid carrier protein (NGALs) levels in patients with severe sepsis and multiple organ dysfunction syndrome (MODSs). Methods Plasma NGAL levels were analyzed in 123 patients with severe sepsis and septic shock. Results MODS was found in 47 cases within 7 days [10 cases had MODS on the first day, 37 cases on the 7th day] and 39 cases died within 12 months. There was a higher risk of MODS and death in the higher plasma NGAL level group than in the lower group. The results of P0.05? ROC curve analysis showed that the plasma NGAL level had predictive value to the death cases in ICU. The regression analysis showed that after adjusting for the mixed risk factors, the plasma NGAL level was significantly higher than that in the lower group. The increase of plasma NGAL level was an independent predictor of HR1. 128 ~ 95 CI 1.078 and 4. 203 P0.05) and MODS HR1. 896 ~ 95% CI 1.012 ~ 3. 552 P 0. 05). Conclusion elevated plasma NGAL level is an independent predictor of MODS and death in patients with severe sepsis.
