【摘要】：目的評價音樂干預對早產(chǎn)兒住院期間生理指標的調(diào)節(jié)作用。方法系統(tǒng)檢索中國知網(wǎng)、萬方數(shù)據(jù)庫、Pub Med、CINAHL、Embase和Cochrane Library數(shù)據(jù)庫從建庫至2017年4月18日的文獻中關于音樂干預對住院早產(chǎn)兒(病情穩(wěn)定、聽力正常)生理指標作用的RCT,干預期間早產(chǎn)兒不接受侵入性刺激、處于平靜狀態(tài),試驗組給予音樂干預(音樂類型、干預時機、次數(shù)和時間不限),對照組不予音樂干預。結局指標為氧飽和度、心率和呼吸頻率。采用JBI文獻質(zhì)量評價工具對納入的RCT進行方法學質(zhì)量評價。采用Rev Man 5.3軟件進行Meta分析。結果 9篇文獻符合本文的納入標準和排除標準,質(zhì)量評價后納入5篇質(zhì)量為A或B的文獻(均為英文)。5篇文獻均報道了氧飽和度,4篇文獻報道了心率和呼吸頻率。Meta分析顯示,試驗組和對照組氧飽和度、心率和呼吸頻率差異均無統(tǒng)計學意義。進一步行亞組分析,根據(jù)音樂干預類型提取搖籃曲干預亞組,根據(jù)音樂實施形式的不同提取錄播音樂亞組和現(xiàn)場演奏音樂亞組,結果顯示,僅錄播音樂干預組氧飽和度高于對照組[MD=0.81,95%CI:0.03~1.59),Z=2.05,P=0.04]。結論音樂干預對調(diào)節(jié)早產(chǎn)兒生理指標的作用不明確,鑒于該領域高質(zhì)量RCT文獻少,需開展更多大樣本高質(zhì)量的RCT進一步探討。
[Abstract]:Objective to evaluate the effect of music intervention on physiological indexes of preterm infants. Methods A systematic search was conducted on the literature on music intervention in the database of Pub MedCINAHL Embase and Cochrane Library from the establishment of the database to April 18, 2017. During the intervention period, premature infants were not subjected to invasive stimulation and were in a calm state. The experimental group was given music intervention (music type, time, times and time), while the control group did not receive music intervention. The outcome indicators were oxygen saturation, heart rate and respiratory rate. The JBI literature quality evaluation tool was used to evaluate the methodological quality of the included RCT. Meta analysis was carried out with Rev Man 5.3 software. Results 9 articles were in accordance with the inclusion and exclusion criteria of this article, and 5 articles with quality A or B were included after quality evaluation (all 5 articles in English) reported oxygen saturation and 4 articles reported heart rate and respiratory frequency. Meta-analysis showed that: 1. There was no significant difference in oxygen saturation, heart rate and respiratory rate between the experimental group and the control group. Further subgroup analysis was carried out to extract the lullaby intervention subgroup according to the type of music intervention, and to extract the music recording subgroup and live music subgroup according to the different forms of music implementation. The results showed that, The oxygen saturation in the music recording intervention group was higher than that in the control group. Conclusion the effect of music intervention on regulating physiological indexes of premature infants is not clear. In view of the lack of high quality RCT literature in this field, it is necessary to further explore the high quality RCT with more large samples.
