【摘要】：目的:探討降鈣素原(procalcitonin,PCT)、C反應(yīng)蛋白(C-reactive protein,CRP)的檢測(cè)在尿膿毒血癥早期診斷過程中臨床價(jià)值,以期對(duì)尿膿毒血癥患者早期診斷,給予及時(shí)有效的干預(yù)措施,改善患者預(yù)后,降低其病死率。方法:收集青海省人民醫(yī)院泌尿外科住院部自2014年01月起至2015年12月間收治的132例尿路感染患者資料,其中診斷為尿膿毒血癥的患者38例為研究組,診斷為非膿毒血癥普通泌尿道感染患者94例為對(duì)照組�；仡櫺苑治鋈脒x患者發(fā)熱早期測(cè)定血清PCT、CRP、白細(xì)胞、中性粒細(xì)胞的水平,兩組數(shù)據(jù)分析比較,并通過統(tǒng)計(jì)學(xué)軟件Medcal對(duì)PCT、CRP等指標(biāo)繪制受試者工作特征曲線(ROC曲線),計(jì)算PCT與CRP的最佳截?cái)嘀导案餮装Y指標(biāo)曲線下面積(area under the curve,AUC),比較各檢測(cè)相關(guān)炎癥指標(biāo)對(duì)早期診斷尿膿毒血癥的敏感度、特異度和準(zhǔn)確性。結(jié)果:尿膿毒血癥組與普通尿路感染組相比較,血清PCT、CRP、白細(xì)胞及中性粒細(xì)胞的水平明顯高(P0.05),差別有統(tǒng)計(jì)學(xué)意義。通過繪制兩組患者PCT與CRP的ROC曲線,PCT與CRP分別取值為1.76ng/ml、12.1mg/L時(shí)兩者AUC分別為0.976、0.861,敏感度分別為95.8%、92.1%,特異度分別為88.3%、43.6%。結(jié)論:1.血清PCT與CRP對(duì)尿膿毒血癥患者早期診斷具有較大的臨床價(jià)值;2.血清PCT與CRP相比較,敏感度和特異度均較高,且AUC值較大,提示PCT在尿膿毒血癥早期診斷的準(zhǔn)確性更高。
[Abstract]:Objective: to investigate the clinical value of the detection of procalcitonin (procalcitonin) C-reactive protein (CRP) in the early diagnosis of urinary sepsis, and to provide timely and effective intervention measures to improve the prognosis of patients with urinary sepsis. Reduce the case fatality rate. Methods: 132 patients with urinary tract infection were collected from January 2014 to December 2015 in Department of Urology, people's Hospital of Qinghai Province. 94 patients with common urinary tract infection were diagnosed as control group. The serum levels of CRP, leukocytes and neutrophils in patients with fever were measured in the early stage of fever, and the data of the two groups were analyzed and compared. The operating characteristic curve of subjects was drawn by statistical software Medcal. The optimum truncation value of PCT and CRP and the area under each inflammatory index curve were calculated. The relative inflammation indexes were compared for early diagnosis. Sensitivity to sepsis, Specificity and accuracy. Results: the serum levels of CRP, leukocytes and neutrophils in urinary sepsis group were significantly higher than those in common urinary tract infection group (P 0.05). By drawing the ROC curve of PCT and CRP in the two groups, the values of ROC and CRP were 1.76 ng / ml / L and 12.1mg / L, respectively. The AUC of the two groups was 0.976mg / L, the sensitivity was 95.8g / 92.1, and the specificity was 88.3mg / L, respectively. Conclusion 1. Serum PCT and CRP have great clinical value in the early diagnosis of urinary sepsis. Compared with CRP, serum PCT had higher sensitivity and specificity, and higher AUC value, which suggested that PCT was more accurate in the early diagnosis of urinary sepsis.
