【摘要】：目的:應(yīng)用靜息態(tài)功能磁共振成像(rfMRI)的低頻振幅比率(fALFF)技術(shù)研究首發(fā)原發(fā)性復(fù)雜部分性發(fā)作癲癇(CPSE)患者在治療前后的腦局部自發(fā)活動(dòng)特征及其與抗癲癇藥物臨床療效的關(guān)系。方法:14例CPSE患者和14例性別、年齡、利手、受教育程度相匹配的健康志愿者(對(duì)照組)進(jìn)行rfMRI檢查,隨后患者組規(guī)律用藥,治療12周后再次行MRI檢查。計(jì)算每例受試者的全腦fALFF值,對(duì)患者組治療前后的數(shù)據(jù)分別與對(duì)照組進(jìn)行雙樣本t檢驗(yàn)。結(jié)果:相比于治療前,CPS患者治療后發(fā)現(xiàn)雙側(cè)丘腦異常激活腦區(qū)消失,雙側(cè)扣帶回、左側(cè)角回異常激活腦區(qū)范圍減小,而左側(cè)頂下小葉激活腦區(qū)的范圍變大,另外發(fā)現(xiàn)于雙側(cè)額葉、楔前葉、左側(cè)楔葉有新的激活腦區(qū)(P0.001,簇大小≥25個(gè)體素)。結(jié)論:fALFF技術(shù)rfMRI可觀察原發(fā)性癲癇患者治療前后的腦功能改變情況,為臨床客觀評(píng)價(jià)治療情況及抗癇藥物的療效提供新的見(jiàn)解,也可為本病的病理生理機(jī)制的進(jìn)一步研究開(kāi)辟新思路。
[Abstract]:Objective: to study the characteristics of local spontaneous brain activity and its relationship with the clinical efficacy of antiepileptic drugs before and after treatment in patients with primary complex partial seizures (CPSEs) using resting functional magnetic resonance imaging (ffMRI) technique with low frequency amplitude ratio (fALFFFF). Methods rfMRI was performed in 14 patients with CPSE and 14 healthy volunteers (control group) who matched sex, age, hands and education level. The patients in the control group were given regular medication. After 12 weeks of treatment, MRI examination was performed again. The whole brain fALFF of each patient was calculated, and the data of the patient group before and after treatment were compared with that of the control group. Results: compared with the patients with CPS before and after treatment, the bilateral thalamus abnormally activated brain region disappeared, bilateral cingulate gyrus decreased the area of abnormal activation of left angular gyrus, and the range of left inferior parietal lobe activated brain area became larger. In addition, a new activated brain area P0.001 was found in bilateral frontal lobe, precuneiform lobe and left cuneate lobe, with cluster size 鈮，
