【摘要】：目的探討布氏桿菌脊柱炎(BS)的MRI表現(xiàn),以提高對該病的認(rèn)識(shí)及診斷水平。方法回顧性分析經(jīng)臨床及實(shí)驗(yàn)室確診或手術(shù)病理證實(shí)的57例BS患者的MRI平掃及增強(qiáng)掃描影像表現(xiàn)。結(jié)果 40例行MRI平掃加增強(qiáng)掃描,17例MRI平掃,累及骶椎10例(17%),腰椎52例(91%),胸椎8例(14%),頸椎3例(5%),"跳躍性"發(fā)病4例(7%)。BS可侵犯脊柱任何部位,以腰椎多見,椎體邊緣"蟲蝕"樣骨質(zhì)破壞,可伴有骨質(zhì)增生,椎體形態(tài)多無明顯變化,椎間盤炎性改變較輕,椎間隙無或輕度狹窄,椎旁、椎管內(nèi)硬膜外膿腫及腰大肌膿腫顯示清楚;大部分病例可見腰背部肌肉腫脹及皮下水腫征象。結(jié)論 MRI影像表現(xiàn)在診斷及鑒別診斷BS方面具有較高的價(jià)值。
[Abstract]:Objective to investigate the MRI findings of Brucella spondylitis (BSS) in order to improve the recognition and diagnosis of the disease. Methods 57 cases of BS confirmed by clinic and laboratory or confirmed by operation and pathology were analyzed retrospectively on MRI plain scan and contrast-enhanced imaging. Results MRI plain scan and enhanced scan were performed in 17 cases, sacral vertebrae were involved in 17 cases, lumbar vertebrae in 52 cases, thoracic vertebrae in 8 cases, cervical spine in 3 cases, and the incidence of "jumping" in 4 cases, 7 cases were involved in any part of the spine, most of them were lumbar vertebrae. The destruction of "wormlike" bone on the edge of the vertebral body may be accompanied by bone hyperplasia, with no obvious changes in the morphology of the vertebral body, less inflammatory changes in the intervertebral disc, no or slight stenosis in the intervertebral space, and a clear display of the epidural abscess and the abscess of the psoas major muscle in the paravertebral, intraspinal epidural and psoas muscles. In most cases, swelling of back muscles and subcutaneous edema were seen. Conclusion MRI imaging is valuable in diagnosis and differential diagnosis of BS.
