【摘要】：目的:評價腺苷脫氨酶(ADA)、結核抗體(TB-Ab)以及結核分枝桿菌DNA(TB-DNA)聯(lián)合檢測在結核性胸膜炎的診斷價值。方法:分別檢測病例組(82例結核性胸膜炎患者)和對照組(80例非結核性胸膜炎患者)胸水中ADA、TB-Ab和TB-DNA指標陽性率,對單一和聯(lián)合檢測結果進行評價。結果:病例組患者ADA、TB-Ab以及TBDNA陽性率均高于對照組,差異有統(tǒng)計學意義(P0.05);ADA、TB-Ab和TB-DNA聯(lián)合檢測的診斷價值最高,靈敏度為87.80%,特異度為83.75%,約登指數(shù)為0.716,陽性預測值為84.71%,陰性預測值為87.01%。結論:ADA、TB-Ab和TB-DNA在結核性胸膜炎患者中的陽性率較高,并且三指標聯(lián)合檢測對結核性胸膜炎具有較高的診斷價值,有助于早期診斷。
[Abstract]:Objective: to evaluate the diagnostic value of adenosine deaminase ADAA, TB antibody TB-Aband Mycobacterium tuberculosis DNA TB-DNA in the diagnosis of tuberculous pleurisy. Methods: the positive rates of ADA-TB-Ab and TB-DNA in pleural effusion of 82 patients with tuberculous pleurisy and 80 patients with non-tuberculous pleurisy in the control group were detected, and the results of single and combined detection were evaluated. Results: the positive rates of ADA-TB-Ab and TBDNA in the case group were higher than those in the control group. The difference was statistically significant (P 0.05). The diagnostic value of TB-Ab combined with TB-DNA was the highest, the sensitivity was 87.80, the specificity was 83.755.The Yorden index was 0.716, the positive predictive value was 84.71, and the negative predictive value was 87.01. Conclusion the positive rate of TB-Ab and TB-DNA in tuberculous pleurisy is higher than that of TB-Ab in tuberculous pleurisy, and the combined detection of three indexes has high diagnostic value for tuberculous pleurisy, which is helpful to the early diagnosis of tuberculous pleurisy.
