【摘要】：目的:探討內(nèi)耳前庭內(nèi)淋巴體積測(cè)量在梅尼埃病中的應(yīng)用價(jià)值。方法:20例經(jīng)臨床確診的梅尼埃病患者經(jīng)鼓膜穿刺雙側(cè)鼓室內(nèi)注入釓噴酸葡胺稀釋液,24h后行MRI掃描,對(duì)內(nèi)耳前庭內(nèi)淋巴體積進(jìn)行測(cè)量。10例為術(shù)前患者,其中7例為單側(cè)發(fā)病患者,3例為雙側(cè)發(fā)病患者;10例為行單側(cè)半規(guī)管填塞術(shù)后患者,其中8例為單側(cè)發(fā)病患者,2例為雙側(cè)發(fā)病患者。采用配對(duì)t檢驗(yàn)比較單側(cè)發(fā)病術(shù)前患者患側(cè)耳與健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積、單側(cè)發(fā)病術(shù)后患者術(shù)后耳與健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積的差異;采用獨(dú)立樣本t檢驗(yàn)比較術(shù)前患者患側(cè)耳與術(shù)后患者術(shù)后耳內(nèi)耳前庭內(nèi)淋巴體積的差異。結(jié)果:7例單側(cè)發(fā)病術(shù)前患者患側(cè)耳和健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積測(cè)量值分別為(11.67±3.63)mm3、(7.99±2.69)mm3,8例單側(cè)發(fā)病術(shù)后患者術(shù)后耳和健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積測(cè)量值分別為(7.54±2.55)mm3、(7.41±2.00)mm3,10例術(shù)前患者患側(cè)耳與10例術(shù)后患者術(shù)后耳內(nèi)耳前庭內(nèi)淋巴體積測(cè)量值分別為(10.93±3.14)mm3、(7.46±2.26)mm3;單側(cè)發(fā)病術(shù)前患者患側(cè)耳和健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積、術(shù)前患者患側(cè)耳與術(shù)后患者術(shù)后耳內(nèi)耳前庭內(nèi)淋巴體積差異有統(tǒng)計(jì)學(xué)意義(P值0.05);單側(cè)發(fā)病術(shù)后患者術(shù)后耳與健側(cè)耳內(nèi)耳前庭內(nèi)淋巴體積差異無統(tǒng)計(jì)學(xué)意義(P值0.05)。結(jié)論:內(nèi)耳前庭內(nèi)淋巴體積測(cè)量能為臨床診斷梅尼埃病提供影像學(xué)依據(jù),同時(shí)為梅尼埃病的療效評(píng)估提供有用的信息。
[Abstract]:Objective: to evaluate the value of intravestibular lymphatic volume measurement in Meniere's disease. Methods Twenty patients with Meniere's disease diagnosed by clinical diagnosis were treated with MRI scan after 24 hours of injection of meglumine gadolinium poumate diluted solution into the tympanic chamber through tympanic membrane puncture. The volume of the vestibular lymph nodes in the inner ear was measured in 10 patients before operation. Among them, 7 cases were unilateral, 3 cases were bilateral, 10 cases were treated with unilateral semicircular canal tamponade, 8 cases were unilateral and 2 cases were bilateral. T test was used to compare the volume of the vestibular lymphatic volume between the patients with unilateral preoperation and that of the patients with unilateral preoperation, and the volume of the vestibular lymphatic volume between the patients after unilateral onset and that of the patients after unilateral operation. T test was used to compare the volume of vestibular lymphoid volume between preoperatively and postoperatively patients with auricular vestibule. Results the volume measurements of the vestibular lymphatic volume in 7 cases of unilateral preoperation and 7 cases of healthy inner ear were 11.67 鹵3.63 mm ~ (3), 7.99 鹵2.69 mm ~ (3) and 7.54 鹵2.55 mm ~ (3) and 7.41 鹵2.00 mm ~ (3) ~ (-1), respectively, before and after operation of unilateral and internal ear of healthy lateral ear were 7.54 鹵2.55 mm ~ (3) and 7.41 鹵2.00 mm ~ (-1), respectively. The volume of vestibular lymphoid volume was 10.93 鹵3.14 mm ~ (3) and 7.46 鹵2.26 mm ~ (3) respectively in the patients with and 10 cases after operation, and the volume of the vestibular lymphoid volume of the affected and healthy ears was 7.46 鹵2.26 mm ~ (3), respectively. There was significant difference in the volume of vestibular lymphatic volume between preoperation and postoperative patients, but there was no significant difference in the volume of vestibular lymph between unilateral and healthy ears. Conclusion: the measurement of intravestibular lymphatic volume of inner ear can provide imaging basis for clinical diagnosis of Meniere's disease and provide useful information for the evaluation of curative effect of Meniere's disease.
