MRI時(shí)間-信號(hào)強(qiáng)度曲線測(cè)量區(qū)域選擇對(duì)乳腺病變的定性診斷價(jià)值

【摘要】:目的探討選擇不同區(qū)域(即病灶中央?yún)^(qū)與邊緣區(qū))測(cè)量MRI時(shí)間-信號(hào)強(qiáng)度曲線(TIC)對(duì)乳腺良惡性病變的鑒別診斷價(jià)值。方法搜集經(jīng)病理證實(shí)的乳腺疾病患者322例(良性142例,惡性180例),共358個(gè)病灶,術(shù)前或活檢前行磁共振動(dòng)態(tài)增強(qiáng)掃描(DCE-MRI);測(cè)量所有病灶中央?yún)^(qū)及邊緣區(qū)TIC類型,統(tǒng)計(jì)分析良、惡性病灶中央?yún)^(qū)與邊緣區(qū)TIC類型分布有無(wú)差異;并采用受試者工作特性曲線(ROC)分析比較病灶中央?yún)^(qū)及邊緣區(qū)TIC類型在不同診斷界值下對(duì)乳腺良惡性病變的鑒別診斷效能。結(jié)果良、惡性病灶中央?yún)^(qū)與邊緣區(qū)TIC類型分布差異均存在統(tǒng)計(jì)學(xué)意義(P0.05)。選擇病灶邊緣區(qū)測(cè)量TIC類型且以診斷界值≥3級(jí)(即Ⅲ型曲線診斷為惡性)作為惡性病變的診斷標(biāo)準(zhǔn),其ROC曲線下面積為0.854,且與選取病灶中央?yún)^(qū)測(cè)量TIC類型診斷乳腺惡性病變的曲線下面積有統(tǒng)計(jì)學(xué)差異(P0.05),診斷敏感度、特異度分別為70.35%、93.08%。結(jié)論選擇病灶不同區(qū)域測(cè)量MRI TIC類型對(duì)乳腺良惡性病變的診斷效能存在差異,選取病灶邊緣區(qū)測(cè)量TIC類型且以Ⅲ型曲線作為惡性病變的診斷標(biāo)準(zhǔn),可以提高對(duì)乳腺良惡性病變的診斷能力。
[Abstract]:Objective to evaluate the differential diagnosis of benign and malignant breast lesions by measuring MRI time-signal intensity curves in different regions (i.e. central and marginal regions). Methods A total of 322 patients (142 benign and 180 malignant) with breast diseases confirmed by pathology were collected. DCE-MRI was performed on dynamic contrast enhanced magnetic resonance imaging before operation or before biopsy, and TIC types in the central and marginal areas of all lesions were measured, and good results were analyzed statistically. There was no difference in the distribution of TIC types between the central and marginal regions of malignant lesions, and the differential diagnostic efficacy of TIC types in the central and marginal regions of the lesions for benign and malignant breast lesions was analyzed and compared by using the operating characteristic curve (OC). Results there were significant differences in the distribution of TIC between the central and marginal regions of benign and malignant lesions (P 0.05).
