【摘要】：目的分析眼弓蛔蟲病(OT)患眼的B型超聲表現(xiàn)及其超聲診斷價值。方法回顧分析50例OT患眼的超聲形態(tài)學(xué)表現(xiàn)及其臨床分型特征。結(jié)果超聲表現(xiàn)包括全視網(wǎng)膜脫離12眼,局限牽引性視網(wǎng)膜脫離6眼,鐮狀視網(wǎng)膜皺褶5眼,玻璃體炎性滲出29眼,增殖條索14眼,球壁局限增厚或中高回聲團(tuán)塊隆起19眼(后極部16眼,周邊部3眼)。臨床分型包括慢性眼內(nèi)炎型16眼,以玻璃體炎性混濁、分層樹枝樣條索、全視網(wǎng)膜脫離為主要超聲表現(xiàn);后部肉芽腫型12眼,以后極部球壁局限增厚、團(tuán)塊隆起、局部牽引性視網(wǎng)膜脫離為主要表現(xiàn);周邊肉芽腫型18眼,以鐮狀視網(wǎng)膜皺褶、周邊牽引性視網(wǎng)膜脫離為主要表現(xiàn);混合型4眼,兼具后部和周邊肉芽腫型特點(diǎn)。結(jié)論 B型超聲能較好反映地OT患眼的各種玻璃體視網(wǎng)膜病變,但易漏診周邊肉芽腫。兒童單眼視網(wǎng)膜局限中高回聲團(tuán)塊伴玻璃體分層樹枝狀條索應(yīng)高度懷疑OT可能。
[Abstract]:Objective to analyze the B-mode ultrasonography and its diagnostic value. Methods the ultrasonic morphology and clinical classification of 50 patients with OT were retrospectively analyzed. Results Ultrasonic findings included total retinal detachment in 12 eyes, limited traction retinal detachment in 6 eyes, sickle retinal fold in 5 eyes, vitreoinflammatory exudation in 29 eyes, proliferative cord in 14 eyes. The bulbous wall was locally thickened or hyperechoic in 19 eyes (16 eyes in the posterior pole and 3 eyes in the periphery). The clinical classification included chronic endophthalmitis in 16 eyes, with vitreous inflammatory opacity, stratified dendritic cord, retinal detachment as the main ultrasonic manifestation, posterior granulomatous type in 12 eyes, the posterior polar bulbar wall was localized thickened, and the bulge was raised. Local traction retinal detachment was the main manifestation; peripheral granulomatous type 18 eyes, sickle retinal fold, peripheral traction retinal detachment as the main manifestation; mixed type 4 eyes, both posterior and peripheral granulomatous characteristics. Conclusion B-mode ultrasound can reflect all kinds of vitreoretinopathy, but it is easy to miss the diagnosis of peripheral granuloma. In children with monocular retinal hyperechoic mass and vitreous stratified dendritic cord, the possibility of OT is highly suspected.
