【摘要】：目的了解鄭州市養(yǎng)老機構(gòu)老年人的關(guān)懷需求現(xiàn)狀并分析其影響因素;進一步挖掘老年人的關(guān)懷需求及影響因素,以期為我國養(yǎng)老機構(gòu)老年人關(guān)懷護理標準的制訂及養(yǎng)老護理人員規(guī)范化培訓方案的構(gòu)建提供有價值參考。方法本研究采用量性研究與質(zhì)性研究相結(jié)合的方法深入探究鄭州市養(yǎng)老機構(gòu)老年人的關(guān)懷需求現(xiàn)狀并分析其影響因素。共包括兩個部分:1.養(yǎng)老機構(gòu)老年人關(guān)懷需求及影響因素的量性研究:隨機抽取鄭州市20所養(yǎng)老機構(gòu),選取其中366名老年人作為研究對象。采用一般資料調(diào)查表、簡易精神認知狀態(tài)問卷(Short Portable Mental Status Questionnaire,SPMSQ)、Barthel指數(shù)評定量表(Barthel Index Scale,BI)和胡慧秀、王志穩(wěn)等于2015年研制的養(yǎng)老機構(gòu)老年人關(guān)懷行為評價量表(Elderly Residents-Perceived Caring Behaviors Inventory),調(diào)查養(yǎng)老機構(gòu)老年人的關(guān)懷需求現(xiàn)狀并分析其影響因素。2.養(yǎng)老機構(gòu)老年人關(guān)懷需求及影響因素的質(zhì)性研究:目的抽樣法,對前期參與調(diào)查的14名老年人進行半結(jié)構(gòu)訪談,深入了解養(yǎng)老機構(gòu)老年人的關(guān)懷需求并挖掘出影響因素。結(jié)果1.研究對象的關(guān)懷需求總得分為(110.99±10.09)分,總均分為(3.96±0.36)分,其中最高分為滿足情感需求維度(4.39±0.61)分,最低分為給予信心和希望維度(3.56±0.58)分。2.以研究對象的關(guān)懷需求總分為因變量,以單因素方差分析有統(tǒng)計學意義(P0.05)的變量:老年人的文化程度、退休前職業(yè)、存活子女數(shù)量、入住時間、居住方式、月收入、醫(yī)保類型、服務(wù)滿意度、ADL狀況為自變量,對無序分類變量設(shè)置啞變量,采用逐步多元回歸法進行分析,結(jié)果顯示:退休前職業(yè)、居住方式、醫(yī)保類型、ADL狀況、服務(wù)滿意度、入住時間6個變量進入了回歸方程,可以解釋總變異的46.5%。3.質(zhì)性研究結(jié)果顯示,養(yǎng)老機構(gòu)老年人有健康服務(wù)、宜居環(huán)境和社會交往方面的關(guān)懷需求,主要影響因素為人員因素和自身因素。結(jié)論鄭州市養(yǎng)老機構(gòu)老年人的關(guān)懷需求總體水平較高且受多種因素影響,養(yǎng)老機構(gòu)在開展相關(guān)服務(wù)時有必要結(jié)合老年人的具體情況并綜合考慮其影響因素,定期評估老年人的關(guān)懷需求,多角度、針對性地為老年人提供安全、專業(yè)、個體化的關(guān)懷照護,滿足其關(guān)懷需求,提高其生活質(zhì)量。
[Abstract]:Objective to understand the present situation of the care needs of the elderly in the aged institutions in Zhengzhou and analyze the influencing factors, and further explore the care needs and influencing factors of the elderly. In order to provide valuable reference for the establishment of care and nursing standard for the elderly and the construction of standardized training program for the aged nursing staff in our country. Methods in this study, quantitative and qualitative studies were used to probe into the present situation of care needs of the elderly in Zhengzhou pension institutions and to analyze its influencing factors. It consists of two parts: one. Quantitative study on the care needs and influencing factors of the elderly in pension institutions: 20 pension institutions in Zhengzhou were randomly selected and 366 of them were selected as research objects. Short Portable Mental Status questionnaire was used to evaluate the Barthel Index scale and Hu Huixiu. Wang Zhi-wen was equal to Elderly Residents-Perceived Caring Behaviors inventory of caring behavior for the elderly developed in 2015. The present situation of care needs of the elderly in pension institutions was investigated and the influencing factors were analyzed. 2. Qualitative study on the care needs and influencing factors of the elderly in pension institutions: objective to conduct semi-structured interviews with 14 older persons who participated in the investigation in the early stage of the investigation, to understand the care needs of the elderly in pension institutions and to find out the influencing factors. Result 1. The total score of care demand was 110.99 鹵10.09, and the total score was 3.96 鹵0.36). The highest score was 4.39 鹵0.61 in the dimension of satisfying emotional needs, and the lowest score was 3.56 鹵0.58 in the dimension of giving confidence and hope. Taking the total score of care needs of the subjects as dependent variables, and using univariate ANOVA as a statistical variable: the educational level of the elderly, pre-retirement occupation, number of surviving children, residence time, living style, monthly income, type of medical insurance, etc. The condition of service satisfaction and ADL is independent variable, the dummy variable of disordered classification variable is set up, and the stepwise multivariate regression method is used to analyze the result. The results show that: occupation before retirement, living style, type of medical insurance, ADL status, service satisfaction, etc. The six variables entered the regression equation, which can explain the total variation of 46.5%. 3. The qualitative research results show that the elderly in old-age institutions have health services, livable environment and social interaction care needs, the main factors are personnel factors and their own factors. Conclusion the overall level of care needs of the elderly in the aged institutions in Zhengzhou is relatively high and affected by many factors. It is necessary for the pension institutions to combine the specific conditions of the elderly and consider the influencing factors when carrying out the related services. To evaluate the care needs of the elderly regularly and to provide them with safe, professional and individualized care from many angles and pertinence, to meet their care needs and to improve their quality of life.
