左室假腱索心電圖早復(fù)極波的對(duì)比分析

【摘要】:目的:研究左室假腱索(LVFT)者早復(fù)極波發(fā)生率,并對(duì)比分析三型早復(fù)極波在LVFT者與正常對(duì)照組的發(fā)生率。方法:收集2016年5月-11月于寧夏人民醫(yī)院心臟彩超室行心臟彩超檢查者共57 6人,年齡1 8-40歲(平均年齡30.75±5.803),其中心臟彩超提示有左室假腱索者276人,無左室假腱索者300人做為對(duì)照組。全部入選者做心電圖檢查并收集一般資料,性別、年齡,癥狀,有無心律失常病史。確定早復(fù)極心電圖,測(cè)量心率、P波時(shí)限,PR間期、QTc、QRS時(shí)限等相關(guān)指標(biāo),分析左室假腱索組及對(duì)照組早復(fù)極波的發(fā)生率,分析三型早復(fù)極波在左室假腱索組及對(duì)照組中分布情況。結(jié)果:(1)收集心臟彩超提示有左室假腱索者共276人,年齡在18-40歲,男性204人(73.9%),女性72人(26.1%),男性左室假腱索發(fā)生率明顯高于女性,差異有統(tǒng)計(jì)學(xué)意義(P0.05),共獲得有效心電圖276份,其中發(fā)生早復(fù)極波98人(35.5%),男性80人(81.6%),女性18人(18.4%);無假腱索者共300人,男性205人(68.3%),女性95人(31.7%),獲取有效心電圖300份,發(fā)生早復(fù)極波36人(12%)。采用卡方檢驗(yàn),左室假腱索組發(fā)生早復(fù)極波明顯高于正常對(duì)照組(P0.05)。(2)左室假腱索組中發(fā)生Ⅰ型早復(fù)極波8人(2.9%),Ⅱ型早復(fù)極波76人(27.5%),Ⅲ型早復(fù)極波14(5.1%),對(duì)照組組中發(fā)生Ⅰ型早復(fù)極波6人(2.0%),Ⅱ型早復(fù)極波24人(17.4%),Ⅲ型早復(fù)極波6人(2.0%),采用卡方檢驗(yàn),Ⅱ型、Ⅲ型早復(fù)極波明顯高發(fā)于左室假腱索組(P0.05,),而Ⅰ型早復(fù)極波在左室假腱索組及正常對(duì)照組中發(fā)生率無差異(P0.05)。(3)對(duì)比左室假腱索組及對(duì)照組兩組人群心電圖,PR間期、QT c均無明顯差異(P0.05);心率、P波時(shí)限、QRS時(shí)限均有統(tǒng)計(jì)學(xué)差異(P0.05),左室假腱索組心率較對(duì)照組慢,P波時(shí)限及QRS時(shí)限左室假腱索組長(zhǎng)于對(duì)照組。結(jié)論:LVFT男性發(fā)生率較女性高;本次研究左室假腱索引起早復(fù)極波發(fā)生率為35.5%;早復(fù)極波發(fā)生以Ⅱ型居多,發(fā)生Ⅲ型早復(fù)極波較正常組高;左室假腱索組人群的P波時(shí)限、QRS時(shí)限均延長(zhǎng)。
[Abstract]:Aim: to study the incidence of early repolarization in patients with left ventricular false chordae tendineae (LVFT) and to compare the incidence of type 3 early repolarization in LVFT patients and normal controls. Methods: a total of 57 6 patients, aged 18 to 40 years (mean age 30.75 鹵5.803), were examined by echocardiography in the heart color Doppler echocardiography room of Ningxia people's Hospital from May to November 2016, among which 276 patients with left ventricular false tendineous chordae were detected by echocardiography. 300 patients without left ventricular false chordae tendineae were used as control group. All participants were examined by electrocardiogram and collected general data, sex, age, symptoms, any history of arrhythmia. To determine the early repolarization electrocardiogram, to measure the heart rate P wave duration and PR interval QTcP QRS time, to analyze the incidence of early repolarization wave in the left ventricular false tendon chorda group and the control group, and to analyze the distribution of the three type early repolarization wave in the left ventricular pseudoaponeurotic chorda group and the control group. Results there were 276 cases (aged 18-40 years) with left ventricular false chordae tendineae revealed by color Doppler echocardiography, 204 males and 26.1 females. The incidence of left ventricular false chordae tendineae in males was significantly higher than that in females, the difference was statistically significant (P 0.05). 276 effective electrocardiograms were obtained. Among them, 98 had early repolarization wave, 80 men had 81.6 right, 18 women 18 had no false tendon chordae, there were 300 cases without false tendon chordae, 205 male with 68.3%, 95 women with 31.775, 300 valid electrocardiograms, and 36 cases with early reversion.
