炎癥指標(biāo)對(duì)血流感染患者病原菌的鑒別診斷評(píng)價(jià)

【摘要】:目的探究炎癥指標(biāo)對(duì)血流感染患者致病菌型別的鑒別診斷效果。方法選取2014年3月-2016年3月于醫(yī)院進(jìn)行住院治療并進(jìn)行血培養(yǎng)表現(xiàn)為陽性診斷的血流感染患者80例,按血培養(yǎng)菌種分為革蘭陽性菌組35例和革蘭陰性菌組42例,分析兩組血流感染患者不同時(shí)期的白細(xì)胞(WBC)、血小板(PLT)、C-反應(yīng)蛋白(CRP)、降鈣素原(PCT)、白細(xì)胞介素-6(IL-6)等指標(biāo),記錄兩組血流感染患者的心率以及體溫,比較兩組患者各項(xiàng)指標(biāo)之間的差異。結(jié)果兩組患者年齡、體質(zhì)量、性別差異均無統(tǒng)計(jì)學(xué)意義;革蘭陰性菌與革蘭陽性菌血流感染患者平均心率與體溫比較無統(tǒng)計(jì)學(xué)差異;革蘭陰性菌與革蘭陽性菌導(dǎo)致的血流感染患者中,在發(fā)病早期其觀察指標(biāo)均有不同程度的改變,其中PLT及CRP的兩組數(shù)據(jù)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),WBC、PCT等觀察指標(biāo)差異無統(tǒng)計(jì)學(xué)意義,革蘭陰性菌組患者WBC為(9.42±5.93)×109/L、PLT為(96.32±45.62)×109/L、CRP為(69.48±38.71ng/mL)、PCT為(4.12±7.81ng/mL)、IL-6為(543.20±1089.42pg/mL),革蘭陽性菌組WBC為(9.11±4.52)×109/L、PLT為(203.76±49.51)×109/L、CRP為(42.84±36.37)ng/ml、PCT為(5.48±12.93)ng/ml、IL-6為(483.17±906.33)pg/ml。結(jié)論在兩組導(dǎo)致的血流感染中,炎癥指標(biāo)可在病癥早期作為快速診療手段,為早期的抗感染及治療提供指導(dǎo),為抗感染方案提供有效的依據(jù),提高血流感染患者的治療效率。
[Abstract]:Objective to investigate the effect of inflammatory markers on the differential diagnosis of pathogenic bacteria in patients with blood flow infection. Methods from March 2014 to March 2016, 80 patients with blood flow infection who were hospitalized and diagnosed by blood culture were divided into Gram-positive bacteria group (35 cases) and Gram-negative bacteria group (42 cases). In order to record the heart rate and body temperature of the two groups of patients with blood flow infection, we analyzed the indexes of WBCU, PLT- C- reactive protein (CRP), procalcitonin (PCT) and interleukin-6 (IL-6) in different stages of blood flow infection, and compared the differences between the two groups. Results there was no significant difference in age, body mass and sex between the two groups, but there was no significant difference in mean heart rate and body temperature between the patients with Gram-negative bacteria and Gram-positive bacteria. In the patients with blood flow infection caused by Gram-negative bacteria and Gram-positive bacteria, the observed indexes were changed in different degree at the early stage of the disease.
