【摘要】：目的探討機構(gòu)照顧者負擔(dān)與老年癡呆患者精神行為癥狀的相關(guān)性及可能的影響因素。方法采用護理院版神經(jīng)精神問卷、照顧者負擔(dān)量表、照顧者積極感受量表、狀態(tài)焦慮量表、抑郁自評量表、智力障礙者照顧困難量表及自制信息采集表對長期護理機構(gòu)112例老年癡呆患者及35名癡呆照顧者進行調(diào)查。結(jié)果機構(gòu)老年癡呆患者的照顧者負擔(dān)總均分(40.57±11.82)高于常模(t=9.20,P=0.00);其中,生理性負擔(dān)(66.94%)和時間依賴性負擔(dān)(62.00%)得分率最高。照顧者焦慮(39.57±8.25)、抑郁(11.68±5.49)自評結(jié)果與國內(nèi)常模無統(tǒng)計學(xué)差異(P0.05)。患者精神行為癥狀嚴重程度與照顧者照顧困難呈正相關(guān)(r=0.40,P=0.04),但二者與照顧者負擔(dān)不相關(guān)(P0.05)。機構(gòu)照顧者對精神行為癥狀的應(yīng)對知識和技能需求最高(89.3%),但因"難以應(yīng)對患者的精神行為癥狀"而離職的意愿最低。結(jié)論長期護理機構(gòu)應(yīng)對老年癡呆患者的照顧者進行精神行為癥狀照護知識和技能培訓(xùn),提高照護質(zhì)量,并采取有效措施,減輕照顧者的生理性和時間依賴性負擔(dān)。
[Abstract]:Objective to investigate the relationship between the burden of institutional caregivers and the symptoms of mental behavior in patients with Alzheimer's disease and the possible influencing factors.Methods Nursing home version neuropsychiatric questionnaire, carer burden scale, caregiver positive feeling scale, state anxiety scale, self rating depression scale were used.A total of 112 patients with Alzheimer's disease and 35 caregivers of dementia in long term nursing institutions were investigated with the Mental retardation scale and self-made information collection table.Results the total mean burden of carers in senile dementia patients was 40.57 鹵11.82), which was higher than that of norm 9.20 P0. 00m, in which the physiological burden was 66.94) and the time-dependent burden was 62.00).There was no statistical difference between the self-evaluation results of anxiety and depression (39.57 鹵8.25, 11.68 鹵5.49) and the national norm (P 0.05).There was a positive correlation between the severity of mental behavior symptoms and caregivers' care difficulties, but no correlation was found between the severity of mental and behavioral symptoms and carers' burden (P 0.05).Institutional caregivers had the highest demand for coping knowledge and skills for mental and behavioral symptoms, but they had the lowest willingness to leave because of the "difficulty in coping with mental and behavioral symptoms of patients".Conclusion Long-term nursing institutions should train caregivers on mental behavior symptom care knowledge and skills, improve the quality of care, and take effective measures to lighten the burden of physiological and time dependence of caregivers.
