【摘要】：目的:非侵入性腦刺激(non-invasive brain stimulation,NIBS)技術(shù)包括重復經(jīng)顱磁刺激(repetitive transcranial magnetic stimulation,r TMS)與經(jīng)顱直流電刺激(transcranial direct current stimulation,tDCS),采用meta分析探討NIBS對卒中后失語癥的治療效果。方法:檢索NIBS治療卒中后失語癥的隨機對照試驗(randomized control trial,RCT),按一定標準、質(zhì)量評價后納入文獻,應用RevMan 5.3軟件進行分析。結(jié)果:最終納入9篇RCT,共214例患者。對照組進行常規(guī)言語訓練,94例;NIBS組進行rTMS或tDCS治療,120例。Meta分析結(jié)果顯示:NIBS治療后,試驗組言語功能評分優(yōu)于對照組[SMD=0.68,95%CI(0.39,0.96),Z=4.66,P0.01];rTMS治療后有評分明顯提高[SMD=0.82,95%CI(0.50,1.14),Z=4.97,P0.01],而tDCS組和對照組治療后差異無顯著性意義[SMD=0.18,95%CI(-0.42,0.79),Z=0.59,P=0.56]。結(jié)論:NIBS技術(shù)對卒中后失語癥患者的言語功能可能有積極作用;r TMS可改善言語功能,而tDCS治療效果有待進一步研究。
[Abstract]:Objective: Non-invasive brain stimulation (NIBS) techniques including repeated transcranial transcranial magnetic stimulation (TMS) and transcranial direct current (TDC) stimulation for transcranial direct current stimulation (TDC) were used to evaluate the therapeutic effect of NIBS on post-stroke aphasia by meta analysis.Methods: a randomized control trialative NIBS trial for the treatment of post-stroke aphasia was searched. According to certain standards, the quality evaluation was included in the literature and analyzed by RevMan 5.3 software.Results: 9 RCTs were included and 214 patients were included.In the control group, 94 patients were treated with rTMS or tDCS. The results of Meta-analysis showed that: 1.Conclusion the effects of TMS on the speech function of patients with aphasia after stroke may be improved by using the technique of TMS. The therapeutic effect of tDCS should be further studied.
