【摘要】：目的探討三維與二維超聲造影在胰腺癌及腫塊型胰腺炎的診斷價(jià)值。方法對(duì)32例胰腺癌、28例腫塊型胰腺炎行二、三維超聲造影檢查,對(duì)比觀察分析60例胰腺腫塊的動(dòng)脈期增強(qiáng)方式、不同時(shí)相的立體感及清晰度、血管清晰度及空間關(guān)系等的差異,比較兩種方法診斷胰腺癌及腫塊型胰腺炎的價(jià)值。結(jié)果 (1)2D、3D-CEUS診斷胰腺癌及腫塊型胰腺炎靈敏度SEN=75.00%、87.50%,特異度SPE=71.43%、78.57%,準(zhǔn)確度ACC=73.33%、83.33%,陽(yáng)性預(yù)測(cè)值+PV=75.00%、82.35%,陰性預(yù)測(cè)值-PV=71.43%、84.61%。(2)3DCEUS對(duì)胰腺腫塊的占位立體感及清晰度、血管清晰度及空間關(guān)系顯示較2D-CEUS具有優(yōu)勢(shì)。結(jié)論 3D-CEUS較2D-CEUS診斷胰腺癌及腫塊型胰腺炎靈敏度、特異度以及準(zhǔn)確度較高,3D-CEUS可顯示病灶整體的立體影像,結(jié)構(gòu)層次和血管空間分布,較2D-CEUS更形象逼真、完整直觀,補(bǔ)充了更多的診斷信息,二者聯(lián)合更可提高胰腺癌與腫塊型胰腺炎的鑒別診斷。
[Abstract]:Objective to evaluate the diagnostic value of three-dimensional and two-dimensional contrast-enhanced ultrasonography in pancreatic cancer and mass pancreatitis.Methods 32 cases of pancreatic carcinoma and 28 cases of mass pancreatitis were examined by two dimensional and three dimensional contrast-enhanced ultrasonography. The contrast analysis of arterial phase enhancement, stereosensory and clarity, vascular clarity and spatial relationship were made in 60 cases of pancreatic mass.To compare the value of the two methods in the diagnosis of pancreatic cancer and mass pancreatitis.Conclusion 3D-CEUS is more sensitive, specific and accurate than 2D-CEUS in the diagnosis of pancreatic cancer and mass pancreatitis. 3D-CEUs can show the stereoscopic image, structure level and vascular spatial distribution of the lesion, and is more vivid, complete and intuitive than 2D-CEUS.In addition to more diagnostic information, the combination of the two can improve the differential diagnosis of pancreatic cancer and mass pancreatitis.
