【摘要】：目的評(píng)價(jià)CURB-65改良評(píng)分對(duì)急診膿毒癥患者預(yù)后的評(píng)估能力。方法回顧性分析2014年1月至2015年1月首診于中國醫(yī)科大學(xué)附屬第一醫(yī)院急診科的143例膿毒癥患者的臨床資料,進(jìn)行CURB-65評(píng)分和序貫器官衰竭評(píng)分(SOFA);以CURB-65評(píng)分為基礎(chǔ),增加與膿毒癥預(yù)后相關(guān)的指標(biāo),形成CURB-65改良評(píng)分,應(yīng)用受試者工作特征曲線分析各評(píng)分系統(tǒng)在膿毒癥預(yù)后中的價(jià)值。結(jié)果在膿毒癥預(yù)后方面,CURB-65改良評(píng)分、CURB-65評(píng)分和SOFA評(píng)分在早期均有獨(dú)立的預(yù)測能力,其中CURB-65改良評(píng)分的曲線下面積(AUC)及約登(Youden)指數(shù)最高(AUC=0.864,Youden指數(shù)=0.581)。CURB-65改良評(píng)分對(duì)早期膿毒癥預(yù)后的預(yù)測能力優(yōu)于傳統(tǒng)的CURB-65評(píng)分及SOFA評(píng)分。結(jié)論 CURB-65改良評(píng)分在早期即可很好地預(yù)測膿毒癥的預(yù)后,且其評(píng)估方法簡單、便捷,可快速評(píng)估急診膿毒癥患者的病情,為早期給予患者有效治療提供幫助。
[Abstract]:Objective to evaluate the ability of evaluating the prognosis of emergency sepsis patients with improved CURB-65 score. Methods from January 2014 to January 2015, the clinical data of 143 patients with sepsis who were first diagnosed in the emergency department of the first affiliated Hospital of China Medical University were analyzed retrospectively. CURB-65 score and Sequential Organ failure score were performed, based on the CURB-65 score, the indexes related to the prognosis of sepsis were increased, and the modified CURB-65 score was formed. Results in the prognosis of sepsis, CURB-65 improved score and CURB-65 score and SOFA score had independent predictive ability in the early stage of sepsis. The area under the curve of CURB-65 improved score and Yorden Youden index are the highest. The modified score of CURB-65 0.864 / Youden index 0.581g 路CURB-65 is superior to the traditional CURB-65 score and SOFA score in predicting the prognosis of early sepsis. Conclusion the modified CURB-65 score can be used in early stage. A good predictor of the prognosis of sepsis, The evaluation method is simple and convenient. It can quickly evaluate the condition of patients with emergency sepsis and provide help for early effective treatment.
