【摘要】：目的探討經(jīng)外周中心靜脈置管(PICC)與植入式中心靜脈輸液港(VPA)對高耐藥性肺癌患者化療中的不良反應(yīng)發(fā)生情況。方法選擇2011年2月至2013年2月哈爾濱醫(yī)科大學(xué)附屬第二醫(yī)院收治診斷為肺癌的患者834例,其中對化療效果欠佳患者行耐藥性分析最終考慮為高耐藥性的患者共56例。將56例高耐藥肺癌患者分成PICC組和VPA組,每組28例。隨訪13個月后,比較兩組患者的導(dǎo)管平均留置時間、導(dǎo)管相關(guān)并發(fā)癥,非導(dǎo)管相關(guān)的臨床并發(fā)癥及生活質(zhì)量、平均生存時間的差異。結(jié)果 VPA組導(dǎo)管平均留置時間及導(dǎo)管相關(guān)、非導(dǎo)管相關(guān)的并發(fā)癥發(fā)生率均明顯低于PICC組(P0.05)。兩組患者的生活質(zhì)量評分比較,差異無統(tǒng)計學(xué)意義(P0.05);但VPA組患者生活質(zhì)量評分A級的比例明顯高于PICC組(P0.05)。兩組患者的平均生存時間比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 VPA在高耐藥性肺癌患者反復(fù)多次化療過程中可作為一種長期留置,此方法方便且安全。
[Abstract]:Objective to investigate the incidence of adverse reactions in chemotherapy of patients with high drug resistance lung cancer treated by PICC and VPA. Methods from February 2011 to February 2013, affiliated to Harbin Medical University, Harbin Medical University was selected. A total of 834 patients with lung cancer were admitted to the second Hospital. 56 patients with high drug resistance were divided into PICC group and VPA group, 28 patients in each group were followed up for 13 months. The differences of average catheter indwelling time, catheter related complications, non-catheter related clinical complications, quality of life and average survival time were compared between the two groups. Results the average catheter retention time and catheter correlation in VPA group were compared. The incidence of non-catheter-related complications was significantly lower than that of PICC group (P 0.05). There was no significant difference (P 0.05), but the proportion of quality of life score A in VPA group was significantly higher than that in PICC group (P 0.05). The mean survival time of the two groups was higher than that of PICC group. Conclusion VPA can be used as a long-term retention during repeated chemotherapy in patients with high resistance lung cancer, and this method is convenient and safe.
