【摘要】：探討肝硬化(liver cirrhosis,LC)患者外周血中血清YKL-40(殼多糖酶3樣蛋白1)蛋白水平及其在LC中的診斷意義。采用ELISA方法共檢測112例LC患者以及114例健康者血清中YKL-40蛋白水平,并進(jìn)一步分析其在LC中的診斷價值及其與LC患者肝功能和現(xiàn)有肝纖維化指標(biāo)的相關(guān)性。LC組血清YKL-40蛋白水平高于正常對照組(P0.001);將LC組和對照組作比較,ROC曲線分析血清YKL-40蛋白對LC的診斷效能,曲線下面積(area under the curve,AUC)為0.934(95%置信區(qū)間為:0.904~0.964),YKL-40在cutoff值為92.25ng/mL時,敏感度為81.3%,特異度為90.4%;通過相關(guān)性分析發(fā)現(xiàn)血清YKL-40蛋白水平與肝功能Child-Pugh分級和FIB-4指數(shù)正相關(guān)。YKL-40對LC具有良好的診斷效力,能輔助診斷LC并有助于判斷LC的嚴(yán)重程度。
[Abstract]:To investigate the level of serum YKL-40 protein in peripheral blood of patients with liver cirrhosis and its diagnostic significance in LC. Serum YKL-40 protein levels were detected by ELISA in 112 patients with LC and 114 healthy controls. The serum YKL-40 protein level in LC group was higher than that in normal control group (P 0.001). The diagnostic efficacy of serum YKL-40 protein for LC. The area under the curve area under the curveAUC = 0.934 ~ 95% confidence interval is:: 0.904 / 0.964ng / mL YKL-40, when cutoff value is 92.25 ng / mL, sensitivity is 81.3 and specificity is 90.40.The positive correlation between serum YKL-40 protein level and liver function Child-Pugh grade and FIB-4 index. YKL-40 has good diagnostic effect on LC. Can assist in the diagnosis of LC and help to judge the severity of LC.
