心率變異性生物反饋干預(yù)不同類(lèi)型失眠障礙的比較

【摘要】:目的探究心率變異性(HRV)生物反饋訓(xùn)練對(duì)不同類(lèi)型失眠障礙患者的影響。方法 2016年6月至2017年3月,對(duì)17例單純性失眠患者、19例失眠伴焦慮患者及19例失眠伴抑郁患者均進(jìn)行HRV生物反饋訓(xùn)練及相同的藥物治療(酒石酸唑吡坦,每晚服用10 mg),訓(xùn)練前后采用匹茲堡睡眠質(zhì)量指數(shù)(PSQI)、癥狀自評(píng)量表(SCL-90)、漢密爾頓焦慮量表(HAMA)及漢密爾頓抑郁量表(HAMD)進(jìn)行評(píng)估并進(jìn)行HRV數(shù)據(jù)采集。結(jié)果 HRV生物反饋訓(xùn)練后,三類(lèi)失眠患者PSQI、SCL-90、HAMA、HAMD評(píng)分及低頻段功率/高頻段功率(LF/HF)均降低(t1.446,P0.05)。訓(xùn)練后,三組PSQI評(píng)分有顯著性差異(F=3.537,P=0.038)。失眠伴焦慮、失眠伴抑郁組訓(xùn)練前后PSQI評(píng)分差值、LF/HF差值大于單純性失眠組(P0.05),失眠伴焦慮與失眠伴抑郁組間差值無(wú)顯著性差異(P0.05)。結(jié)論 HRV生物反饋訓(xùn)練對(duì)三種類(lèi)型失眠患者均有改善作用,其中,對(duì)失眠伴焦慮及失眠伴抑郁組患者睡眠質(zhì)量改善程度優(yōu)于單純性失眠患者。
[Abstract]:Objective to investigate the effect of biofeedback training on different types of insomnia in patients with heart rate variability (HRV). Methods from June 2016 to March 2017, HRV biofeedback training and the same drug therapy (zolpidem tartrate, Zolpidem tartrate) were performed in 17 patients with simple insomnia, 19 patients with insomnia and anxiety and 19 patients with insomnia and depression. Before and after training, the Pittsburgh Sleep quality Index (PSQI), symptom Checklist-90 (SCL-90), Hamilton anxiety scale (Hama) and Hamilton Depression scale (Hamd) were used to evaluate and collect HRV data before and after training. The scores of Hama Hamd and low frequency power / high frequency power LF / HF decreased in three types of insomnia patients (PSQI / SCL-90). After training, there was a significant difference in PSQI scores among the three groups (P = 3.537a, P = 0.038). The difference of PSQI score before and after training in insomnia with depression group was higher than that in simple insomnia group (P 0.05). There was no significant difference between insomnia with anxiety and insomnia with depression. Conclusion HRV biofeedback training can improve all three types of insomnia. Among them, the sleep quality of insomnia with anxiety and insomnia with depression was better than that of simple insomnia.
