【摘要】：目的評價經(jīng)導(dǎo)管動脈栓塞術(shù)(TAE)治療口鼻腔大出血的效果、并發(fā)癥發(fā)生及預(yù)防。方法回顧2005年11月至2013年10月經(jīng)治的121例難治性及致命性口鼻腔大出血患者臨床資料。對116例接受TAE術(shù)治療患者分別進行1~3個月隨訪,了解栓塞效果及相關(guān)并發(fā)癥發(fā)生情況。結(jié)果 116例患者中96例(82.7%)出血完全控制,19例(16.4%)1周內(nèi)再次出血并經(jīng)藥物治療得以控制,1例(0.9%)1周內(nèi)再次出血接受二次栓塞治療;77例(66.4%)無明顯并發(fā)癥發(fā)生,35例(30.1%)出現(xiàn)頜面部疼痛、麻木,低熱,張口受限等輕度并發(fā)癥,1例(0.9%)出現(xiàn)面部皮膚壞死及重度頭痛,3例(2.6%)因腦栓塞出現(xiàn)卒中癥狀。結(jié)論根據(jù)不同病因及責(zé)任血管選擇適宜栓塞材料,TAE術(shù)治療難治性及致命性口鼻腔大出血可快速有效達(dá)到止血目的。術(shù)后常見并發(fā)癥有栓塞后綜合征、局部缺血、末梢循環(huán)缺血所致局部壞死,嚴(yán)重并發(fā)癥主要為異位栓塞所致頜面部皮膚壞死、腦梗死。
[Abstract]:Objective to evaluate the value of transcatheter arterial embolization (TAE) treatment of oral nasal bleeding effect, complications and prevention. Methods 121 cases from November 2005 to 2013 10 menstrual treatment of refractory and lethal oral nasal bleeding in patients with clinical data of 116 patients underwent TAE. Patients were followed up for 1~3 months, understand the effect of embolism and related complications. Results 116 patients in 96 cases (82.7%) hemorrhage control completely, 19 cases (16.4%) within 1 weeks after drug treatment and re bleeding was controlled, 1 cases (0.9%) within 1 weeks of bleeding again received two embolization; 77 cases (66.4%) without obvious complications (30.1%), 35 cases appeared facial pain, numbness, fever, trismus mild complications, 1 cases (0.9%) had facial skin necrosis and severe headache, 3 cases (2.6%) with cerebral embolism stroke symptoms. Conclusion according to different causes and responsibility to choose suitable vessels Embolic material, TAE surgery in the treatment of refractory and lethal oral nasal bleeding can effectively achieve hemostasis. The common postoperative complications after embolization syndrome, ischemia, peripheral circulation ischemia induced local necrosis, serious complications mainly for ectopic embolism caused by maxillofacial skin necrosis, cerebral infarction.
