關(guān)于青少年速滑運(yùn)動(dòng)員膝關(guān)節(jié)磁共振定量參數(shù)變化的研究

【摘要】:目的:假設(shè)速滑運(yùn)動(dòng)能引起青少年髕軟骨及半月板組織結(jié)構(gòu)的改變,導(dǎo)致MRI定量參數(shù)的變化,研究青少年速滑運(yùn)動(dòng)員與正常青少年膝關(guān)節(jié)髕骨軟骨與半月板之間T2 mapping、T2 star及Resolve DWI定量參數(shù)的差異是否有統(tǒng)計(jì)學(xué)意義。方法:研究對(duì)象:將隨機(jī)抽取的某中學(xué)初二年級(jí)某班級(jí)的39名學(xué)生納入正常對(duì)照組,將隨機(jī)抽取的某速度滑冰訓(xùn)練基地28名青少年速度滑冰運(yùn)動(dòng)員納入實(shí)驗(yàn)組。采用3.0T磁共振(Mgnetom Skyra system,Siemens),膝關(guān)節(jié)專用線圈,仰臥位,足先進(jìn),對(duì)研究對(duì)象進(jìn)行掃描。由2位有5年膝關(guān)節(jié)MRI閱片經(jīng)驗(yàn)的影像診斷醫(yī)師采用雙盲法進(jìn)行數(shù)據(jù)測(cè)量,測(cè)量所得數(shù)據(jù)采用SPSS 19.0進(jìn)行統(tǒng)計(jì)學(xué)分析。數(shù)據(jù)進(jìn)行正態(tài)分布檢測(cè),若符合正態(tài)分布,用平均數(shù)±標(biāo)準(zhǔn)差表示,兩組間采用獨(dú)立樣本t檢驗(yàn)進(jìn)行統(tǒng)計(jì)學(xué)分析;若不符合正態(tài)分布,用中位數(shù)表示,采用非參數(shù)檢驗(yàn)進(jìn)行統(tǒng)計(jì)學(xué)分析。組內(nèi)兩兩比較均采用q檢驗(yàn)。P㩳0.05認(rèn)為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果:1.正常組髕軟骨T2值(24.02±4.25)、ADC值(1158.49±203.63)低于實(shí)驗(yàn)組(29.07±7.17)、(1578.48±294.28),P㩳0.05,差異有統(tǒng)計(jì)學(xué)意義;正常組髕軟骨T2*值(22.71±3.24)低于實(shí)驗(yàn)組(23.72±5.95),P0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義。2.正常組內(nèi)側(cè)半月板前角T2值(18.64±3.88)、ADC值(773.53±272.46)低于實(shí)驗(yàn)組(22.92±4.82)、(1178.60±234.80),P㩳0.05,差異有統(tǒng)計(jì)學(xué)意義;實(shí)驗(yàn)組內(nèi)側(cè)半月板前角T2*值(5.98±2.26)低于正常組(6.83±2.28),P0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義。3.正常組內(nèi)側(cè)半月板后角T2值(17.37±6.57)、ADC值(741.75±159.19)低于實(shí)驗(yàn)組(21.41±3.76)、(1130.44±268.39),P㩳0.05,差異有統(tǒng)計(jì)學(xué)意義;正常組內(nèi)側(cè)半月板后角T2*值(7.85±3.30)低于實(shí)驗(yàn)組(8.30±2.34),P0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義。4.實(shí)驗(yàn)組外側(cè)半月板前角T2值(19.27±4.70)低于正常組(19.55±7.05),正常組外側(cè)半月板前角T2*值(6.39±1.96)、ADC值(734.01±252.72)低于實(shí)驗(yàn)組(6.67±1.66)、(857.21±243.89),P0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義。5.正常組外側(cè)半月板后角T2值(15.26±2.47)、T2*值(6.90±1.58)、ADC值(881.00±114.00)低于實(shí)驗(yàn)組(17.16±4.24)、(8.54±5.99)、(1026.11±258.75),P0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論:1.青少年速滑運(yùn)動(dòng)員與正常青少年的髕軟骨T2值和ADC值存在差異。2.青少年速滑運(yùn)動(dòng)員與正常青少年的內(nèi)側(cè)月板前、后角T2值和ADC值存在差異。
[Abstract]:Objective: to assume that speed skating can cause changes in the tissue structure of patellar cartilage and meniscus in adolescents and lead to the change of quantitative parameters of MRI. To study the difference of quantitative parameters of T 2 mapping T 2 star and Resolve DWI between juvenile speed skaters and normal juvenile knee joint patella cartilage and meniscus. Methods: study participants: a middle school junior high school selected randomly. Thirty-nine students in a class in a grade were included in the normal control group. Twenty-eight young speed skaters were randomly selected from a certain speed skating training base to be included in the experimental group with 3.0T magnetic resonance imaging (MRI) of Mgnetom Skyra system Siemens, a special coil for the knee joint, supine position, advanced foot, and so on. Two imaging diagnostics who had 5 years experience in reading MRI film of knee joint were used to measure the data by double blind method. The measured data were analyzed statistically by SPSS 19.0. The data were detected by normal distribution. If the normal distribution is consistent with the mean 鹵standard deviation, the independent sample t test is used for statistical analysis between the two groups; if it does not conform to the normal distribution, the median is used. A non-parametric test was used for statistical analysis. Results the T _ 2 value of patellar cartilage in normal group was 24.02 鹵4.25 and ADC value was 1158.49 鹵203.63), which was lower than that in experimental group (29.07 鹵7.17) and 1578.48 鹵294.28 (P < 0.05). The T _ 2 * value of patellar cartilage in the normal group was 22.71 鹵3.24), which was lower than that in the experimental group (23.72 鹵5.95), no significant difference was found. 2. The T _ 2 value of the medial meniscus anterior horn of the normal group was 18.64 鹵3.88 鹵272.46) and the ADC value of the medial meniscus of the normal group was lower than that of the experimental group (22.92 鹵4.82). The T _ 2 * value of the medial meniscus in the experimental group was 5.98 鹵2.26), which was lower than that in the normal group (6.83 鹵2.28), and there was no significant difference. 3. The T _ 2 value of the medial meniscus in the normal group was 17.37 鹵6.57 鹵159.19) and the ADC value of the medial meniscus was lower than that of the experimental group (21.41 鹵3.760.1144 鹵268.39). The difference was statistically significant. The T _ 2 * value of the medial meniscus in the normal group was 7.85 鹵3.30), which was lower than that in the experimental group (8.30 鹵2.34), the difference was not statistically significant. The T _ 2 value of the lateral meniscus in the experimental group was 19.27 鹵4.70), and the T _ 2 * value of the anterior angle of the lateral meniscus in the normal group was 6.39 鹵1.96 鹵252.72), which was lower than that in the experimental group (6.67 鹵1.667.21 鹵243.89P0.05), and the ADC value of the lateral meniscus in the normal group was lower than that in the experimental group (6.67 鹵1.667.21 鹵243.89P0.05). There was no statistical significance. The T 2 value of the lateral meniscus in the normal group was 15.26 鹵2.47 T2 * and the ADC value was 881.00 鹵114.00, which was lower than that in the experimental group (17.16 鹵4.24), which had no statistical significance. Conclusion there is no statistical significance between the T 2 value and ADC value of patellar cartilage between juvenile skaters and normal adolescents. Young speed skaters and normal teenagers in front of the medial meniscus, There was a difference between T _ 2 and ADC in the posterior horn.
