【摘要】：目的探索合并心血管疾病(CVD)的透析患者的病死率及死亡危險因素。方法選取廣州軍區(qū)廣州總醫(yī)院及廣州市第一人民醫(yī)院共591例透析患者,采集基線人口統(tǒng)計學資料、合并癥及實驗室指標等,通過Cox回歸模型探討合并CVD的透析患者的病死率及死亡危險因素。結(jié)果共納入終末期腎病患者591例,其中合并CVD的透析患者123例(CVD組),無CVD的透析患者468例(無CVD組)。CVD組患者平均年齡(59.37±15.94)歲。在總?cè)巳褐?CVD組患者病死率要高于無CVD組患者(HR=2.893,95%CI:1.804~4.693,P0.001)。多因素分析發(fā)現(xiàn),腦卒中(HR=3.802,95%CI:1.752~8.252,P=0.001)為CVD組的獨立死亡危險因素,糖尿病(HR=2.974,95%CI:1.569~5.668,P=0.001)及高敏C反應蛋白(HR=1.107,95%CI:1.005~1.030,P=0.005)為無CVD組的獨立死亡危險因素。結(jié)論合并CVD的透析患者病死率高于無CVD的透析患者,且腦卒中是合并CVD的透析患者的死亡獨立危險因素。
[Abstract]:Objective to explore the patients with cardiovascular disease (CVD) mortality of patients on dialysis rate and risk factors of death. Methods a total of 591 cases in Genenral Hospital of PLA Guangzhou Military Area and Guangzhou No.1 People's Hospital dialysis patients, collected baseline demographics, comorbidities and laboratory indicators such as mortality, through the Cox regression model to explore the combined CVD patients on dialysis rate and risk factors of death were included in the results. In patients with end-stage renal disease in 591 cases, including 123 cases with CVD dialysis patients (CVD group), 468 patients with non dialysis CVD patients (CVD group) the average age of patients in group.CVD (59.37 + 15.94) years old. In the general population, the mortality rate of patients in group CVD was higher than that of non CVD patients (HR=2.893,95%CI:1.804~4.693, P0.001). Multivariate analysis showed that stroke (HR=3.802,95%CI:1.752~8.252, P=0.001) CVD group is the independent risk factors of death in diabetes (HR=, 2.974,95%CI:1.569~5.668, P=0.001) and high C, HR=1.107,95%CI:1.005~1.030 (P=0.005) is an independent risk factor of death in group CVD. Conclusion the mortality of patients with CVD combined with dialysis is higher than that of patients without CVD, and stroke is an independent risk factor for death of patients with CVD combined with dialysis.
