【摘要】：目的研究前列腺核磁共振成像(MRI)聯(lián)合前列腺特異性抗原密度(PSAD)對前列腺癌(PCa)和前列腺增生(BPH)的診斷價值。方法 81例有穿刺活檢或手術病理結果的前列腺病例,經穿刺或手術病理確診28例為前列腺癌患者,53例為前列腺增生患者,回顧分析其資料,收集患者MRI的診斷結果、前列腺特異性抗原(PSA)數(shù)值及測量前列腺體積,結合PSA數(shù)值及前列腺體積,計算出PSAD,分別分析前列腺MRI及PSAD診斷結果及當MRI診斷不明時,聯(lián)合PSAD的診斷結果。結果 MRI對前列腺癌和前列腺增生的診斷準確率分別為90.0%(18/20)、96.0%(24/25);PSAD對前列腺癌和前列腺增生的診斷準確率分別為68.0%(17/25)、80.4%(45/56);當MRI診斷不明確時,聯(lián)合PSAD,前列腺癌陰性診斷率為78.6%(22/28)。結論聯(lián)合MRI和PSAD可以明顯提高前列腺癌和前列腺增生的診斷水平。
[Abstract]:Objective to study the diagnostic value of prostatic magnetic resonance imaging (MRI) combined with prostate specific antigen density (PSAD) in the diagnosis of prostatic carcinoma (PCA) and prostatic hyperplasia (BPH). There were 28 cases of prostate cancer diagnosed by puncture or operation and pathology. The data of 28 patients with prostate cancer and 53 patients with prostatic hyperplasia were analyzed retrospectively, the diagnostic results of MRI, the value of prostate specific antigen (PSA) and the volume of prostate were collected. Combined with PSA value and prostate volume, PSAD was calculated, and the diagnostic results of MRI and PSAD were analyzed, respectively, and when the diagnosis of MRI was unknown, Results the diagnostic accuracy of MRI for prostate cancer and prostatic hyperplasia was 90.0 / 20 / 20 / 96. 0, respectively. The diagnostic accuracy of MRI for prostate cancer and prostatic hyperplasia was 68.0 / 25 / 80.4 / 4556, respectively. When the diagnosis of MRI was not clear, In combination with PSAD, the negative diagnosis rate of prostate cancer was 78.6 / 28%. Conclusion the combination of MRI and PSAD can significantly improve the diagnostic level of prostate cancer and prostatic hyperplasia.
