【摘要】:目的:了解初產(chǎn)婦母乳喂養(yǎng)自我效能的現(xiàn)狀,探討基于信息動機行為技巧模型的護理干預對初產(chǎn)婦母乳喂養(yǎng)自我效能的影響,為提高初產(chǎn)婦母乳喂養(yǎng)自我效能提供科學合理的參考依據(jù)。方法:選擇2016年1月至2016年6月期間,在吉林大學第一醫(yī)院產(chǎn)科待產(chǎn)的120例初產(chǎn)婦作為研究對象,其中2016年1月~3月的60例初產(chǎn)婦作為對照組,2016年4月~6月的60例初產(chǎn)婦作為試驗組。對照組接受產(chǎn)科常規(guī)護理措施,試驗組接受4個月的基于信息動機行為技巧模型的護理干預。產(chǎn)前進行基線調(diào)查,產(chǎn)后第42天及產(chǎn)后第4個月評價干預效果。采用母乳喂養(yǎng)自我效能簡式量表評價初產(chǎn)婦的母乳喂養(yǎng)自我效能;應用母乳喂養(yǎng)知識問卷對初產(chǎn)婦的母乳喂養(yǎng)知識進行評測;利用母乳喂養(yǎng)行為問卷結(jié)合微信、電話等方式調(diào)查初產(chǎn)婦的母乳喂養(yǎng)行為及純母乳喂養(yǎng)率。結(jié)果:1.基線資料顯示:兩組初產(chǎn)婦的一般基本資料、母乳喂養(yǎng)自我效能、母乳喂養(yǎng)知識得分比較均無統(tǒng)計學差異(P0.05)。2.干預對母乳喂養(yǎng)自我效能的影響:組間比較結(jié)果顯示,產(chǎn)后第42天及產(chǎn)后第四個月試驗組初產(chǎn)婦的母乳喂養(yǎng)自我效能總分、技能和個人內(nèi)心維度得分高于對照組,兩組比較有統(tǒng)計學差異(P0.05);自我效能總分及各維度得分的干預主效應和時間主效應均有統(tǒng)計學差異(P0.05),干預與時間之間存在交互效應,隨著時間的延長,產(chǎn)后第42天兩組自我效能總分及各維度得分均較產(chǎn)前有所提高,但試驗組分數(shù)提升幅度較大(P0.05),產(chǎn)后第四個月實驗組自我效能總分及各維度得分均提高,但對照組自我效能總分和技巧得分較產(chǎn)后第42天有所下降。組內(nèi)比較結(jié)果顯示,對照組干預前后3個時間點的母乳喂養(yǎng)自我效能總分及各維度得分比較均無統(tǒng)計學差異(P0.05);試驗組干預前后3個時間點的母乳喂養(yǎng)自我效能總分及各維度得分比較均有統(tǒng)計學差異(P0.05)。3.干預對母乳喂養(yǎng)知識、行為的影響:產(chǎn)后第42天及產(chǎn)后第四個月試驗組母乳喂養(yǎng)知識及行為得分均高于對照組,兩組比較有統(tǒng)計學差異(P0.05)。4.干預對純母乳喂養(yǎng)率的影響:產(chǎn)后第42天及產(chǎn)后第四個月兩組喂養(yǎng)方式比較有統(tǒng)計學差異(P0.05),試驗組純母乳喂養(yǎng)率高于對照組。結(jié)論:1.基于信息動機行為技巧模型制定的護理措施能給初產(chǎn)婦提供母乳喂養(yǎng)方面的支持和幫助,從而提高其母乳喂養(yǎng)自我效能。2.基于信息動機行為技巧模型的護理干預能提升初產(chǎn)婦的母乳喂養(yǎng)知識水平,改善初產(chǎn)婦的母乳喂養(yǎng)行為。3.基于信息動機行為技巧模型的護理干預能提高純母乳喂養(yǎng)率,延長母乳喂養(yǎng)時間。
[Abstract]:Objective: to understand the current situation of breast-feeding self-efficacy in primipara, and to explore the effect of nursing intervention based on information-motivated behavioral skills model on breast-feeding self-efficacy of primipara. Methods: from January 2016 to June 2016, 120 primiparous women in the first Hospital of Jilin University were selected as the research objects, in order to improve the self-efficacy of breast-feeding. From January 2016 to March, 60 primiparas were used as control group and 60 primipara cases from April 2016 to March as trial group. The control group received routine obstetric nursing measures. The trial group received four months of nursing intervention based on information motivational behavioral skills model. Baseline prenatal surveys were conducted. The intervention effect was evaluated on the 42nd day postpartum and the 4th month postpartum. The breast-feeding self-efficacy was evaluated by breast-feeding self-efficacy scale and the breast-feeding knowledge questionnaire was used to evaluate the breast-feeding knowledge of primipara. The breast-feeding behavior and exclusive breast-feeding rate of primipara were investigated by using the questionnaire of breast-feeding behavior and WeChat, etc. Results: the baseline data showed that: the general basic data of the two groups of primipara, the self-efficacy of breast-feeding, and so on. There was no statistical difference in the scores of breast feeding knowledge. 2. The effect of intervention on breast feeding self-efficacy: the results of comparison between groups showed that the total score of breast-feeding self-efficacy of primipara on the 42nd day after delivery and the fourth month postpartum. The scores of skills and individual inner dimension were higher than those of the control group, there were significant differences between the two groups (P 0.05), and there were significant differences in the total scores of self-efficacy and the scores of each dimension between the two groups (P 0.05), and there was a significant difference in the main effect of intervention and the main effect of time in each dimension, and there was an interactive effect between intervention and time. With the extension of time, the total score of self-efficacy and the scores of each dimension in the two groups were improved compared with those before delivery on the 42nd day of postpartum, but the scores of the experimental group increased significantly (P0.05), and the total score of self-efficacy and the scores of each dimension of the experimental group increased in the fourth month of postpartum. However, the total scores of self-efficacy and skills of the control group were lower than those of the 42nd day postpartum. There was no significant difference in the total score and dimension scores of breast feeding self-efficacy between the control group and the control group at three time points before and after intervention, and the comparison between the three time points of the trial group before and after intervention and the score of each dimension. There was significant difference between the two groups (P 0.05). The knowledge of intervention on breast feeding, The influence of behavior: the scores of knowledge and behavior of breast-feeding in the experimental group were higher than those in the control group on the 42nd day and the fourth month postpartum. The effect of intervention on the rate of exclusive breastfeeding: there was significant difference between the two groups on the 42nd day of postpartum and the fourth month of postpartum. The rate of exclusive breast-feeding in the trial group was higher than that in the control group. Nursing measures developed by the information motivational behavioral skills model can provide breast-feeding support and assistance to primiparous women. Therefore, nursing intervention based on information motivational behavioral skills model can improve the knowledge level of breast-feeding of primipara. To improve breast-feeding behavior of primipara. Nursing intervention based on information-motivated behavioral skills model can increase exclusive breastfeeding rate and prolong breast-feeding time.
