亞急性期腦梗死的體素內(nèi)不相干運(yùn)動(dòng)擴(kuò)散加權(quán)成像

【摘要】:目的探討體素內(nèi)不相干運(yùn)動(dòng)(IVIM)成像評(píng)價(jià)亞急性期腦梗死彌散和灌注情況的價(jià)值。方法回顧性分析35例亞急性期腦梗死患者的MR影像資料,包括常規(guī)DWI、IVIM成像及三維動(dòng)脈自旋標(biāo)記(3D-ASL)序列檢查。通過常規(guī)DWI序列獲得ADC圖,IVIM序列獲得純水分子擴(kuò)散系數(shù)(D)圖、灌注相關(guān)擴(kuò)散系數(shù)(D*)圖、灌注分?jǐn)?shù)(f)圖,3D-ASL序列獲得腦血流量(CBF)圖。分別測(cè)量腦梗死灶及對(duì)照側(cè)鏡像區(qū)ADC值、D值、D*值、f值、CBF值及其相對(duì)值(rADC、rD、rD*、rf、rCBF)。比較腦梗死灶與對(duì)照側(cè)鏡像區(qū)各參數(shù)的差異,并分析rADC值與rD、rD*、rf值的相關(guān)性及rCBF值與rD、rD*、rf值的相關(guān)性。結(jié)果腦梗死灶A(yù)DC、D、D*、f、CBF值均較對(duì)照側(cè)鏡像區(qū)減低(P均0.05)。rADC值與rD、rf值呈正相關(guān)(r=0.687、0.601,P=0.005、0.018);rCBF值與rf值呈正相關(guān)(r=0.581,P=0.022)。結(jié)論 IVIM成像能同時(shí)提供亞急性期腦梗死的灌注及彌散信息,有助于進(jìn)一步闡明亞急性期腦梗塞的機(jī)制。
[Abstract]:Objective to evaluate the diffusion and perfusion of subacute cerebral infarction by voxel incoherent motion imaging (IVIMI). Methods 35 cases of subacute cerebral infarction were retrospectively analyzed. Including routine DWI IVIM imaging and 3D arterial spin labeling 3D-ASL sequence examination. ADC map was obtained by routine DWI sequence. IVIM sequence obtained pure water molecular diffusion coefficient (D) map, perfusion correlation diffusion coefficient (D) map and perfusion fraction (f) map. The cerebral blood flow (CBF) map was obtained by 3D-ASL sequence. The ADC value and D value, D value, D value and relative value of ADC in the cerebral infarction area and the mirror image region of the control side were measured respectively. To compare the difference of the parameters between the cerebral infarction focus and the mirror image area of the control side, and to analyze the correlation between the rADC value and the rDdfr value, and the rCBF value and the rDd * value. Results the CBF values of cerebral infarction were significantly lower than those of the control group (P < 0.05). The rf value was positively correlated with 0.6870.601P0. 005 and 0. 018m. There was a positive correlation between rCBF value and RF value. Conclusion IVIM imaging can provide information of perfusion and diffusion of subacute cerebral infarction at the same time. It is helpful to further elucidate the mechanism of subacute cerebral infarction.
