【摘要】：目的探討超聲心動(dòng)圖對(duì)肺動(dòng)脈吊帶的診斷價(jià)值。方法回顧性分析23例肺動(dòng)脈吊帶超聲心動(dòng)圖二維圖像、彩色多普勒聲像圖表現(xiàn)并與心血管造影、心臟CT及手術(shù)結(jié)果對(duì)比分析,總結(jié)其診斷特點(diǎn),找出漏診的原因。結(jié)果術(shù)前確診19例,漏診4例。2例合并法洛四聯(lián)癥,3例合并動(dòng)脈導(dǎo)管未閉,3例合并房間隔缺損,2例合并永存左上腔靜脈。超聲的特征性表現(xiàn)為肺動(dòng)脈分叉消失,主肺動(dòng)脈直接延續(xù)為右肺動(dòng)脈,于右肺動(dòng)脈發(fā)出異常起源的左肺動(dòng)脈。漏診病例中:2例合并法洛四聯(lián)癥,1例合并動(dòng)脈導(dǎo)管未閉。23例患兒均伴有不同程度氣道受壓,有明顯氣道狹窄的18例。19例超聲診斷與心臟CT及手術(shù)結(jié)果符合。結(jié)論超聲心動(dòng)圖診斷肺動(dòng)脈吊帶具有重要臨床意義。
[Abstract]:Objective to evaluate the value of echocardiography in the diagnosis of pulmonary artery sling. Methods Twenty three cases of pulmonary artery sling were analyzed retrospectively by echocardiography, color Doppler sonography and angiocardiography. Results 19 cases were diagnosed before operation, and 4 cases were missed in 4 cases with tetralogy of Fallot 3 cases with patent ductus arteriosus. Left superior vena cava was found in 2 cases with atrial septal defect in 3 cases. The characteristic manifestation of ultrasound was that the bifurcation of pulmonary artery disappeared and the main pulmonary artery continued directly to the right pulmonary artery. The left pulmonary artery with abnormal origin in the right pulmonary artery. In the misdiagnosed cases, 2 cases were complicated with tetralogy of Fallot, 1 case with patent ductus arteriosus. 23 cases were all accompanied with different degree of airway pressure. The ultrasonic diagnosis of 18 cases with obvious airway stenosis was consistent with the results of cardiac CT and operation. Conclusion Echocardiography is of great clinical significance in the diagnosis of pulmonary artery sling.
