雙相情感障礙患者靜息態(tài)功能磁共振Meta分析

【摘要】:目的:雙相情感障礙(Bipolar disorder,BD)是一種高致殘、高疾病負(fù)擔(dān)的重性精神障礙,靜息態(tài)下低頻振幅(Amplitude of low frequency fluctuation,ALFF)以及局部一致性(Regional homogeneity,ReHo)異常提示雙相情感障患者腦功能活動異常。本研究系統(tǒng)評價雙相情感障礙患者與健康對照組靜息態(tài)功能磁共振低頻振幅及局部一致性的差異,旨在探索雙相情感障礙患者神經(jīng)影像學(xué)標(biāo)志物。方法:計算機(jī)檢索Pubmed、Embase、Ovid、Web of Science、EBSCOhost medline、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(CBMdisc)、CNKI、VIP以及萬方數(shù)據(jù)庫,收集關(guān)于雙相情感障礙患者及健康對照組靜息態(tài)功能磁共振ALFF及ReHo的病例對照研究。按系統(tǒng)評價方法,由兩名研究者獨立進(jìn)行質(zhì)量評價和資料提取,采用Anisotropic Effect-Size-Based Algorithms(AES-SDM)(內(nèi)置P0.005,體素k=10)進(jìn)行坐標(biāo)預(yù)處理、異質(zhì)性檢驗、敏感度分析、閾值檢驗、元回歸分析,得出差異腦區(qū)生成結(jié)果圖像。結(jié)果:本研究共納入8篇文獻(xiàn),其中ALFF相關(guān)文獻(xiàn)4篇,樣本量為206例,其中病例組及健康對照組各103例。Meta分析研究結(jié)果顯示,與健康對照相比,病例組ALFF值增高的腦區(qū)有:左側(cè)豆?fàn)詈?左前丘腦,右側(cè)額下回;ALFF值降低的腦區(qū)有:左側(cè)舌回,左側(cè)小腦區(qū)域(半球小葉Ⅰ,IV/V,梭回)。ReHo相關(guān)文獻(xiàn)4篇,樣本量為155例,其中病例組75例,健康對照相80例。Meta分析研究結(jié)果顯示,與健康對照相比,病例組ReHo值增高的腦區(qū)有:右側(cè)杏仁核,右側(cè)海馬,左尾狀核;ReHo值降低的腦區(qū)有:右側(cè)顳上回,左側(cè)前額回,額內(nèi)側(cè)回。結(jié)論:雙相情感障礙患者在左側(cè)基底核區(qū)域、右側(cè)額下回、左側(cè)舌回及左側(cè)小腦區(qū)域存在ALFF值異常,在右側(cè)杏仁核及海馬區(qū)域,左尾狀核、右側(cè)顳上回、左側(cè)前額回、額內(nèi)側(cè)回存在ReHo值異常,提示雙相情感障礙的發(fā)病可能與這些腦區(qū)局部功能異常有關(guān)。
[Abstract]:Objective: bipolar disorder (BDD) is a kind of mental disorder with high disability and high burden of disease. Low frequency amplitude of low frequency fluctuation. ALF) and regional homogeneity. This study systematically evaluated the differences of resting FMR low frequency amplitude and local consistency between bipolar affective disorder patients and healthy controls. Objective to explore neuroimaging markers in patients with bipolar affective disorder (bipolar disorder). Methods: Pubmedmedus Embase Ovidof Science was searched by computer. EBSCOhost Medline, Chinese Biomedical Literature Database (CBM) and Chinese Biomedical Literature Database (EBSCOhost) and Wanfang database. A case-control study on resting functional magnetic resonance (ALFF) and ReHo in patients with bipolar disorder and healthy controls was conducted. The quality evaluation and data extraction were carried out by two researchers independently. Anisotropic Effect-Size-Based algorithm AES-SDM (built-in P0.005) is used. Coordinate preprocessing, heterogeneity test, sensitivity analysis, threshold test and meta-regression analysis were performed to obtain the resulting image of differential brain region. Results: eight articles were included in this study. Among them, there were 4 articles related to ALFF, the sample size was 206 cases, among them, the case group and the healthy control group were 103 cases each. Meta-analysis results showed that compared with the healthy control group. In the case group, the ALFF increased in the left lentiform nucleus, the left anterior thalamus and the right inferior frontal gyrus. The ALFF decreased in the left lingual gyrus and the left cerebellar area (I / V in the hemispheric lobules I / V, 4 articles in the gyrus fusifolia. ReHo). The sample size was 155 cases, including 75 cases in the case group. The results of meta analysis of 80 cases of healthy radiographs showed that the ReHo increased in the right amygdaloid nucleus, the right hippocampus and the left caudate nucleus in the case group compared with the healthy control group. ReHo decreased in the right superior temporal gyrus, left prefrontal gyrus, medial frontal gyrus. Conclusion: bipolar affective disorder patients in the left basal nucleus region, right inferior frontal gyrus. In the right amygdala and hippocampus, the left caudate nucleus, the right superior temporal gyrus, the left prefrontal gyrus and the medial frontal gyrus were abnormal. The results suggest that the pathogenesis of bipolar disorder may be related to the abnormal local function of these brain regions.
