【摘要】：目的通過調(diào)查糖尿病患者相關(guān)心理痛苦的現(xiàn)狀,分析糖尿病患者相關(guān)心理痛苦與自我管理行為、社會(huì)支持、應(yīng)對(duì)方式的相關(guān)性;從中介機(jī)制角度探討社會(huì)支持、應(yīng)對(duì)方式在糖尿病患者相關(guān)心理痛苦與自我管理行為之間的中介效應(yīng),為實(shí)施切實(shí)有效的干預(yù)策略,提升患者生活質(zhì)量提供理論依據(jù)。方法采用方便抽樣法選取銀川市3所三級(jí)甲等綜合醫(yī)院內(nèi)分泌科2016年4月至2016年10月住院的糖尿病患者240例為研究對(duì)象。采用一般資料問卷、糖尿病相關(guān)心理痛苦量表(DDS)、糖尿病自我管理活動(dòng)量表(SDSCA)、社會(huì)支持評(píng)定量表(SSRS)、醫(yī)學(xué)應(yīng)對(duì)方式問卷(MCMQ)進(jìn)行橫斷面調(diào)查。數(shù)據(jù)經(jīng)收集整理后進(jìn)行統(tǒng)計(jì)描述、Logistic回歸分析和中介效應(yīng)分析。結(jié)果1.240例調(diào)查對(duì)象中,伴有糖尿病相關(guān)心理痛苦者163例,檢出率為67.92%,包括中等痛苦者101例,嚴(yán)重痛苦者62例。2.單因素分析及Logistic回歸分析結(jié)果顯示:家庭人均月收入低、自費(fèi)、病程短、有糖尿病并發(fā)癥、患有其他疾病、不能堅(jiān)持糖尿病管理、自我管理行為得分低、社會(huì)支持得分低以及選擇屈服應(yīng)對(duì)方式的患者更易發(fā)生糖尿病相關(guān)心理痛苦。3.糖尿病患者相關(guān)心理痛苦各維度與自我管理行為中的血糖監(jiān)測(cè)和足部護(hù)理顯著負(fù)相關(guān),與客觀支持、主觀支持顯著負(fù)相關(guān),與屈服顯著正相關(guān)。4.社會(huì)支持在糖尿病相關(guān)心理痛苦和自我管理行為之間具有中介作用。結(jié)論糖尿病患者發(fā)生相關(guān)心理痛苦的現(xiàn)象較為普遍;文化程度、家庭人均月收入、醫(yī)療付費(fèi)方式、病程、是否患有糖尿病并發(fā)癥、是否患有其他疾病、糖尿病治療同時(shí)是否接受其他病治療、能否堅(jiān)持糖尿病管理、自我管理行為、社會(huì)支持、應(yīng)對(duì)方式均會(huì)影響糖尿病患者相關(guān)心理痛苦的發(fā)生;社會(huì)支持在糖尿病患者相關(guān)心理痛苦與自我管理行為之間具有中介作用。
[Abstract]:Objective to analyze the correlation between psychological pain and self-management behavior, social support and coping style by investigating the current situation of diabetes patients' related psychological pain. From the perspective of intermediary mechanism, this paper discusses the intermediary effect of social support and coping style between diabetes patients' related psychological pain and self-management behavior, in order to implement effective intervention strategies. Methods the convenient sampling method was used to select 240 diabetic patients hospitalized in the Endocrinology Department of 3 Grade 3A General Hospital in Yinchuan from April 2016 to October 2016. General information questionnaire was used. The Diabetes Mellitus related Psychological pain scale (DDSU), the Diabetes Self-Management activity scale (DMS) and the Social support rating scale (SSRS). Medical coping style questionnaire (MCMQ) was used to conduct cross-sectional survey. Logistic regression analysis and intermediary effect analysis. Results 1. 67.92% of the 67.92% subjects were accompanied with diabetes related psychological pain. The results of univariate analysis and Logistic regression analysis showed that the average monthly income of family was low, self-expense, short course of disease and complications of diabetes. With other diseases, can not adhere to diabetes management, self-management behavior score is low. Patients with low scores of social support and the choice of coping styles were more likely to develop diabetes related psychological pain. 3. The dimensions of diabetes related psychological pain were significantly associated with blood glucose monitoring and foot care in self-managed behavior. Negative correlation. There was a significant negative correlation between subjective support and objective support. Social support plays an intermediary role between diabetes related psychological pain and self-management behavior. Conclusion the phenomenon of diabetes related psychological pain is more common in diabetic patients. Education level, per capita monthly income, medical payment, course of disease, whether there are diabetes complications, whether there are other diseases, diabetes treatment at the same time whether to accept other diseases, whether to adhere to diabetes management. Self-management behavior, social support, coping style will affect the occurrence of psychological pain in patients with diabetes. Social support plays a mediating role between diabetes related psychological pain and self-management behavior.
