【摘要】:目的探討術(shù)前MRI預(yù)測經(jīng)皮椎體強(qiáng)化術(shù)(PVA)中骨水泥椎間盤滲漏的臨床價值。方法回顧性分析經(jīng)PVA治療的127例骨質(zhì)疏松性椎體壓縮性骨折(OVCF)患者的臨床及影像資料,比較患者年齡、性別、手術(shù)椎體部位、手術(shù)方式及術(shù)前MRI是否存在骨折椎體終板損傷、達(dá)終板骨折線、鄰近椎間盤損傷及椎體裂隙時骨水泥椎間盤滲漏率的差異。結(jié)果 127例OVCF患者中,手術(shù)椎體179個,涉及358個終板及341個椎間盤。有終板損傷、達(dá)終板骨折線、鄰近椎間盤損傷及椎體裂隙征象的骨水泥鄰近椎間盤滲漏率分別為57.73%(56/97)、60.98%(25/41)、35.91%(51/142)、55.56%(20/36),均高于無上述征象椎體(P均0.05)。經(jīng)皮椎體后凸成形術(shù)(PKP)及經(jīng)皮椎體成形術(shù)(PVP)患者椎體骨水泥椎間盤滲漏率分別為26.67%(16/60)、42.86%(51/119),兩者差異具有統(tǒng)計學(xué)意義(P=0.035)。骨水泥注入量≤5ml椎體骨水泥椎間盤滲漏率為31.19%(34/109),低于骨水泥注入量5ml椎體[47.14%(33/70),P=0.031)]。性別、年齡、手術(shù)椎體部位對鄰近椎間盤骨水泥滲漏的影響無統(tǒng)計學(xué)意義(P0.05)。結(jié)論術(shù)前MRI能有效預(yù)測PVA術(shù)中骨水泥向鄰近椎間盤滲漏,選擇PKP及控制骨水泥注入量等方式可以降低骨水泥椎間盤滲漏的風(fēng)險。
[Abstract]:Objective to evaluate the clinical value of preoperative MRI in predicting the leakage of bone cement disc in percutaneous vertebra augmentation. Methods 127 cases of osteoporotic vertebral compression fracture treated with PVA were retrospectively analyzed. Clinical and imaging data of patients with OVCFS. To compare the age, sex, site of operation vertebral body, operation method and MRI before operation whether there is fracture vertebral body endplate injury, to the end plate fracture line.
