兒科ICU護(hù)士對患兒父母支持狀況與其需求滿足程度的相關(guān)性研究

【摘要】:目的:調(diào)查了解兒科ICU護(hù)士對患兒父母的支持狀況,分析患兒父母需求滿足程度的現(xiàn)狀,評價(jià)護(hù)士的支持能否滿足患兒父母的需求,并分析影響護(hù)士對患兒父母支持及患兒父母需求滿足程度的因素,探討兩者之間的相關(guān)性,針對存在的問題提出相應(yīng)的建議及護(hù)理措施,從而為“以家庭為中心的護(hù)理”模式在兒科的應(yīng)用提供參考依據(jù)。方法:于2016年07月至2016年09月,采取方便抽樣法,選擇山東省及吉林省共4所醫(yī)院的兒科ICU住院患兒的父母210名,對其進(jìn)行問卷調(diào)查。問卷有一般資料調(diào)查表、護(hù)士對患兒父母支持量表及需求滿足程度量表三個(gè)部分。運(yùn)用Epi Data3.1開展數(shù)據(jù)錄入與管理,運(yùn)用SPSS19.0對數(shù)據(jù)統(tǒng)計(jì)處理。一般資料采用描述性分析;運(yùn)用t檢驗(yàn)、單因素方差分析或Kruskal-Wallis,H檢驗(yàn)分析不同性別、年齡、家庭月收入等的患兒父母獲得的護(hù)理支持程度和需求滿足程度是否存在差異;采用Pearson或Spearman分析法對兩者之間的關(guān)聯(lián)性進(jìn)行探討,將一般資料中有統(tǒng)計(jì)學(xué)意義的變量作為自變量,以患兒父母的需求滿足程度作為因變量進(jìn)行多元逐步回歸分析,討論影響兒科ICU患兒父母需求滿足程度的因素。結(jié)果:1.本研究共計(jì)發(fā)放問卷210份,回收問卷206份,有效問卷199份,問卷回收率為98.10%,有效回收率為94.76%。2.兒科ICU護(hù)士對患兒父母支持總得分為(65.67±15.82)分,四個(gè)維度的條目平均得分按從高到低順序依次為:優(yōu)質(zhì)護(hù)理照顧支持維度、溝通與信息支持維度、情感支持維度、尊重支持維度。3.患兒父母的需求滿足程度總分為(131.12±24.23)分,五個(gè)維度條目平均得分按從高到低順序依次為:病情保證、獲取信息、親近患者、自我舒適、醫(yī)護(hù)人員的支持。4.單因素分析結(jié)果表明,年齡、文化程度、家庭月收入、醫(yī)療費(fèi)用支付方式、家庭居住環(huán)境不同,護(hù)士對患兒父母的支持水平差異有統(tǒng)計(jì)學(xué)意義(P0.05);住院時(shí)間、有無宗教信仰、文化程度、家庭月收入、醫(yī)療費(fèi)用支付方式及家庭居住環(huán)境的差別,對患兒父母的需求滿足程度得分差異有統(tǒng)計(jì)學(xué)意義(P0.05)。5.多元逐步回歸分析結(jié)果表明,護(hù)理支持總分、有無宗教信仰、住院時(shí)間(15~30天)、文化程度(本科及以上)進(jìn)入患兒父母需求滿足程度得分的回歸方程,并解釋總變異量的52.6%。6.相關(guān)性分析顯示,護(hù)士對患兒父母的支持與其需求滿足程度及各維度呈正相關(guān)(r=0.568,P<0.01)。結(jié)論:1.兒科ICU護(hù)士對患兒父母的支持處于中等水平,需要進(jìn)一步提高,其得分高低的影響因素主要有患兒父母年齡、文化程度、家庭月收入、醫(yī)療費(fèi)用支付方式、家庭居住環(huán)境。2.患兒父母需求滿足程度位于中等偏上水平,影響得分的因素有:住院時(shí)間、宗教信仰、文化程度、家庭月收入、醫(yī)療費(fèi)用支付方式及家庭居住環(huán)境。3.護(hù)士對患兒父母的支持與其需求滿足程度呈正相關(guān),護(hù)士對患兒父母的支持程度越高,患兒父母的需求滿足程度越高。提高護(hù)士的職業(yè)素養(yǎng)和綜合素質(zhì),加強(qiáng)兒科以家庭為中心的優(yōu)質(zhì)護(hù)理服務(wù)理念,完善患兒父母的支持系統(tǒng),有助于改善其身心狀態(tài),提高對護(hù)理的依從性和對護(hù)理職業(yè)的認(rèn)同,以此保障醫(yī)護(hù)服務(wù)質(zhì)量的提升和患兒的康復(fù),提高患兒父母的需求滿足程度,改善護(hù)患關(guān)系。
[Abstract]:Objective: To investigate the pediatric ICU nurses in their parental support, parents need to meet the level of analysis in the status quo, evaluation of nurse support can meet the needs of parents of children, and to analyze the influence of nurses on the parents support and parents demand satisfaction factors, to explore the relationship between them, aiming at the existing problems, suggestions and the corresponding nursing measures, so as to provide reference for family centered care model in the pediatric application. Methods: from 2016 07 to 2016 09 months, by the method of convenient sampling, choose Shandong province and Jilin Province, a total of 4 hospitals of the pediatric ICU parents of hospitalized children 210, questionnaire survey on the questionnaire. Results: 1. of the 210 questionnaires, 206 questionnaires, 199 valid questionnaires. The questionnaire recovery rate was 98.10%, the effective recovery rate of 94.76%.2. for pediatric ICU nurses in their parental support for the total score (65.67 + 15.82) points, four dimensions of the average entry According to the score from high to low order: high quality nursing care support dimensions, communication and information support dimension, dimension of emotional support, respect and support dimensions of parents of children with.3. demand satisfaction score was (131.12 + 24.23), the five dimensions of entry according to average scores from high to low order: the condition that gets information to patients, self comfort, single factor staff support the results of.4. analysis showed that age, educational level, family income, payment of medical expenses, the family living environment is different, the difference was statistically significant for parents of children with the support level of nurses (P0.05); the length of time, there is no religion, culture degree. Family income, medical expenses payment of living environment and family differences were statistically significant in their parental demand satisfaction scores (P0.05) and.5. multivariate regression analysis results show that support Physical support, there is no religious beliefs, hospitalization time (15~30 days), education (undergraduate and above) to meet the needs of parents in children with regression equation score, 52.6%.6. correlation analysis and explain the variance show that nurses support for parents of children with their demand satisfaction and dimensions were positively correlated (r=0.568, P < 0.01). Conclusion: 1. pediatric ICU nurses to support parents in the middle level, need to be further improved, factors affecting the scores are the age of the parents, children education, family income, payment of medical expenses, the family living environment to meet the needs of parents of children with.2. level in the medium level, influence factors score: hospitalization time, religious beliefs, cultural degree, family income, payment of medical expenses and family living environment to meet the.3. nurse children and their parents need support The degree of positive correlation, the higher the degree of support of nurses on the parents of children with parents, to meet the needs of the higher level. Occupation accomplishment and improve the overall quality of nurses, strengthen the pediatric family centered nursing quality service concept, improve the support system of parents of children, help to improve their physical and mental state, improve the compliance of nursing and the nursing occupation identity, in order to protect and enhance the quality of medical service with the rehabilitation, improve the satisfaction degree of the parents needs, improve the nurse patient relationship.
