【摘要】：目的:探討廣西少數(shù)民族地區(qū)20-80歲健康人群血管內(nèi)皮功能的影響因素,以及應(yīng)用高分辨率超聲探頭檢測血管內(nèi)皮功能的診斷價值。方法:(1)選取長期居住在廣西柳州市融安縣及融水縣20-80歲的健康人群共749人作為研究對象,已排除經(jīng)臨床診斷患有冠心病、腦卒中、外周動脈疾病、腎臟疾病患者;已排除患有糖尿病和高血壓的人群;已排除月經(jīng)期、孕期及哺乳期婦女。(2)收集內(nèi)容涉及問卷、一般體格檢查包括身高體重并計算體重指數(shù),以及應(yīng)用血管內(nèi)皮功能檢測儀檢測右上肢肱動脈血流介導(dǎo)的舒張功能(Flow-mediated dilation,FMD)、反應(yīng)性充血前后的肱動脈血管內(nèi)徑、血流速度及反應(yīng)性充血血后血管內(nèi)徑及血流速度出現(xiàn)變化的時間。結(jié)果:(1)單因素分析顯示:FMD在不同民族、年齡組、性別和吸煙情況差異有統(tǒng)計學(xué)意義,而在不同體重指數(shù)及喝酒情況差異無統(tǒng)計學(xué)意義。(2)Logistic回歸分析顯示:民族、年齡、性別是FMD的獨立影響因素。苗族的FMD較其他民族較高,老年組的FMD明顯低于青年組及中年組,女性的FMD高于男性。(3)相關(guān)分析表明FMD與肱動脈基礎(chǔ)內(nèi)徑、充血反應(yīng)后最大內(nèi)徑、安靜時血流速及充血反應(yīng)后最大血流速明顯負相關(guān),與擴張達峰時間及充血反應(yīng)后最大血流速出現(xiàn)時間明顯正相關(guān);而與充血后血流速反應(yīng)時間無明顯相關(guān)性。結(jié)論:(1)血管內(nèi)皮功能存在民族差異性,其中苗族的FMD較高,漢族的FMD較其他少數(shù)民族低,因此應(yīng)重視民族差異對血管內(nèi)皮功能的影響。(2)同時,年齡及性別差異與血管內(nèi)皮功能有關(guān)。(3)超聲檢測指標(biāo)可有效評估血管內(nèi)皮功能及其變化。
[Abstract]:Objective: To investigate the factors affecting the vascular endothelial function in healthy population aged 20-80 in Guangxi minority area, the diagnostic value and application of high resolution ultrasound probe detection of vascular endothelial function. Methods: (1) choosing long-term healthy people living in the county and Rongshui County of Guangxi at the age of 20-80 in Liuzhou a total of 749 people as the research object, has been ruled out by the clinical diagnosis of coronary heart disease, stroke, peripheral arterial disease, patients with kidney disease were excluded; diabetes and hypertension crowd; have excluded the menstrual period, pregnancy and breast-feeding women. (2) to collect related content and questionnaire, physical examination including height and weight and body mass index, diastolic function and vascular endothelial function detection of right upper limb brachial artery flow mediated (Flow-mediated, dilation, FMD), brachial artery diameter after reactive hyperemia, blood flow velocity and blood reactive hyperemia After the vessel diameter and blood flow velocity changes in time. Results: (1) single factor analysis showed that FMD in different ethnic groups, age groups, there were significant gender and smoking status differences in different body mass index and drinking was no significant difference (2). Logistic regression analysis showed that: age, nationality, sex is the independent factors of FMD. The higher than other ethnic Miao FMD, elderly group FMD was significantly lower than that of the young group and middle-aged group, female FMD than men (3). Correlation analysis showed that FMD and brachial arteries, after the reaction hyperemia diameter, blood velocity and maximum blood flow after quiet congestive reaction speed is obviously negative correlation, positive correlation with the maximum blood flow velocity time dilation and congestion by time after reaction; no significant correlation with blood flow response time after patients. Conclusion: (1) vascular endothelial function of ethnic differences, including seedling The FMD of the ethnic group is higher. The FMD of Han nationality is lower than that of other ethnic groups. Therefore, we should pay attention to the influence of ethnic differences on vascular endothelial function. (2) at the same time, age and gender differences are related to vascular endothelial function. (3) ultrasonography can effectively evaluate endothelial function and its changes.
