【摘要】：目的探討血栓彈力圖(thromboelastography,TEG)與凝血試驗各項指標(biāo)的相關(guān)性,評價兩種方法在指導(dǎo)心臟手術(shù)患者圍術(shù)期輸血中的意義。方法分析本院2014年9月—2016年8月收治的108位心臟手術(shù)患者,隨機分為TEG檢測指導(dǎo)輸血組(實驗組)和凝血試驗指導(dǎo)輸血組(對照組),分別就兩組的輸血率、不同成分血的輸血量及輸血效果進行比較。結(jié)果對TEG和凝血試驗指導(dǎo)心臟手術(shù)圍術(shù)期用血率的比較,發(fā)現(xiàn)試驗組指導(dǎo)冷沉淀25.9%和新鮮冰凍血漿33.3%的輸注率明顯低于對照組的68.5%,而血小板的輸注率57.4%卻明顯高于對照組的20.4%;兩組紅細(xì)胞的輸注率差異無統(tǒng)計學(xué)意義,但紅細(xì)胞的輸注量試驗組(3.6+1.6)明顯低于對照組(5.8+1.9),試驗組指導(dǎo)的新鮮冰凍血漿和冷沉淀的輸注量也明顯低于對照組(P0.05);兩組術(shù)中出血量、術(shù)后出血量、再次出血率方面,試驗組均低于對照組(P0.05)。結(jié)論血栓彈力圖指導(dǎo)心臟手術(shù)患者圍術(shù)期輸血可減少血液制劑的使用量,并對疾病的預(yù)后轉(zhuǎn)歸優(yōu)于常規(guī)凝血試驗指導(dǎo)的用血,減少手術(shù)并發(fā)癥及出血量,對指導(dǎo)臨床成分輸血具有重要意義,值得推廣。
[Abstract]:Objective to investigate the correlation between thromboelastography (TEG) and various indexes of coagulation test, and to evaluate the significance of two methods in guiding perioperative blood transfusion in patients undergoing cardiac surgery. Methods 108 patients undergoing cardiac surgery in our hospital from September 2014 to August 2016 were selected, randomly divided into TEG detection guide blood transfusion group (experimental group) and blood coagulation test guide blood transfusion group (control group), blood transfusion volume and the effect of each of the two groups of different component blood transfusion rates, in comparison. The results of comparison with the rate of TEG and blood coagulation test for cardiac surgery, found that the test group guidance 25.9% cryoprecipitate and fresh frozen plasma 33.3% infusion rate was significantly lower than the control group 68.5%, and the platelet infusion rate 57.4% was significantly higher than the control group 20.4%; injection rate between the two groups of red blood cell transfusion there was no statistical significance, but the red blood cell transfusion test group (3.6+1.6) was significantly lower than the control group (5.8+1.9), the test group guidance of fresh frozen plasma and cryoprecipitate infusion volume was significantly lower than the control group (P0.05); the two group of intraoperative bleeding, postoperative bleeding, rebleeding rate and the test group were lower than the control group (P0.05). Conclusion Thromboelastograms guidance in patients undergoing cardiac surgery perioperative blood transfusion can reduce the amount of blood preparation, and prognosis of the disease outcome is better than the conventional coagulation test guided by blood, reduce complications and blood loss, is of great significance to guide the clinical transfusion of blood components, worthy of promotion.
