【摘要】：研究目的本研究通過梳理老年人口慢性病社區(qū)管理相關(guān)理論,總結(jié)分析國內(nèi)外相關(guān)經(jīng)驗,以重慶市為實證調(diào)研對象,對重慶市社區(qū)衛(wèi)生服務(wù)機構(gòu)管理老年慢性病患者的現(xiàn)狀進(jìn)行定量與定性研究。通過綜合分析社區(qū)衛(wèi)生服務(wù)機構(gòu)人力資源、衛(wèi)生服務(wù)效率以及量表數(shù)據(jù),研究重慶市社區(qū)老年人口慢性病防控管理的可行性及策略,為社區(qū)老年慢性病管理的相關(guān)研究提供理論與實踐參考依據(jù)。資料來源通過檢索數(shù)據(jù)庫,查閱相關(guān)書刊資料,檢索搜索引擎等方式收集社區(qū)老年人口慢性病管理的相關(guān)文獻(xiàn)、研究報告及信息,并收集政府衛(wèi)生、財政、民政、統(tǒng)計等部委官方網(wǎng)站相關(guān)法律法規(guī)、政策文件、年度報表等統(tǒng)計數(shù)據(jù)和資料。通過自行設(shè)計的調(diào)查量表對各樣本區(qū)進(jìn)行調(diào)研獲取實證資料。調(diào)研組共計發(fā)放700份調(diào)查問卷,最終回收675份有效問卷。研究方法采用文獻(xiàn)研究法、問卷調(diào)查法、德爾菲法、統(tǒng)計分析法等。研究結(jié)果重慶市共有12個區(qū)縣成功創(chuàng)建為國家級的慢性病防控示范區(qū),在慢性病防控管理方面有一定成效,開展了家庭醫(yī)生簽約服務(wù),將慢性病管理納入各區(qū)縣的年度社會事業(yè)發(fā)展規(guī)劃中,對居民進(jìn)行健康教育。但在過程中也存在一些的問題:(1)慢性病防控專項經(jīng)費不足,籌資渠道單一;(2)社區(qū)衛(wèi)生人力資源配置不合理,技術(shù)水平參差不齊;(3)部門之間缺乏合作,防控體系不完善;(4)信息系統(tǒng)建設(shè)滯后,不利于慢性病防控管理信息的共享。對策建議(1)政府應(yīng)制定有效的慢性病管理政策,建立規(guī)范的管理體系,調(diào)整財政支出結(jié)構(gòu),加大對社區(qū)老年人口慢性病管理的經(jīng)費投入。(2)加強慢性病管理相關(guān)部門的合作,與科研機構(gòu)保持合作關(guān)系,促進(jìn)科研成果轉(zhuǎn)化為實際成果,社區(qū)應(yīng)重點關(guān)注重慶市慢性病管理薄弱的老年人群。(3)創(chuàng)建健康的生活環(huán)境,提倡健康的生活方式,加強老年慢性病患者的自我管理意識。
[Abstract]:This study by reviewing the theory of elderly population of chronic diseases in community management, summarize relevant experience at home and abroad, taking Chongqing city as the object of empirical research, quantitative and qualitative research on the status quo of management of the elderly in Chongqing community health service institutions for chronic diseases. Through the human resources comprehensive analysis of community health services, health services and efficiency the amount of data, the feasibility and strategy of Chongqing city community elderly population of chronic disease prevention and control and management research, provides a theoretical and practical reference for related research management of senile chronic diseases. The data sources through searching the database, access to relevant books, literature retrieval search engine to collect the elderly population in the community chronic disease management, research report and information, and the collection of government health, civil affairs, finance, statistics and other ministries official website of the relevant laws and regulations, policy paper Annual report, statistics and data. Through the questionnaire designed by the research on the table the sample area to obtain empirical data. The research group issued a total of 700 questionnaires, 675 valid questionnaires. The final research method adopts the literature research method, questionnaire survey method, Del Faye method, statistical analysis and other research. Results a total of 12 Chongqing counties successfully created a national chronic disease prevention and control demonstration area, is effective in the prevention and control of chronic disease management, carry out the family doctor service, the management of chronic diseases included in the planning and development of social undertakings in the districts of the year, to carry out health education for residents. But there are also some of the the problems in the process: (1) chronic disease prevention and control of special funds, financing channels; (2) the community health human resource allocation is not reasonable, technical level is uneven; (3) the lack of cooperation between departments, prevention and control system Perfect; (4) the lag of information system construction, information sharing is not conducive to the management of chronic disease prevention and control countermeasures. (1) the government should formulate effective policies for chronic disease management, establish a standardized management system, adjust the structure of financial expenditure, increase investment in the management of chronic diseases in community elderly population funds. (2) to strengthen chronic disease the management departments and maintain cooperative relations with scientific research institutions, scientific research achievements into practical results, the community should focus on the elderly people in Chongqing chronic disease management is weak. (3) to create a healthy living environment, advocate healthy lifestyle, strengthen the elderly patients with chronic disease self management awareness.
