動(dòng)脈瘤性蛛網(wǎng)膜下腔出血(aSAH)臨床數(shù)據(jù)庫軟件研發(fā)

【摘要】:目的:動(dòng)脈瘤性蛛網(wǎng)膜下腔出血是臨床上常見的急性腦血管疾病,其年發(fā)生率達(dá)6~35.3/10萬,是僅次于腦血栓和高血壓腦出血的腦血管疾病,具有高致死率和致殘率。目前在研究其臨床特點(diǎn)、影響因素、干預(yù)因素、流行病學(xué)和預(yù)后等相關(guān)領(lǐng)域時(shí)產(chǎn)生了大量的臨床數(shù)據(jù),這些大數(shù)據(jù)需要整合、歸納和分析,并且需要一個(gè)較為完整的數(shù)據(jù)庫來錄入、整合和提取所需的數(shù)據(jù)。從當(dāng)前醫(yī)療行業(yè)領(lǐng)域信息化水平的特點(diǎn)來看,任何醫(yī)學(xué)科技創(chuàng)新與醫(yī)療技術(shù)發(fā)展都離不開臨床數(shù)據(jù)的支持,臨床數(shù)據(jù)庫的建立顯得尤為重要,因此需要建立一個(gè)智能化的動(dòng)脈瘤性蛛網(wǎng)膜下腔出血(a SAH)臨床數(shù)據(jù)庫,為臨床與科研提供數(shù)據(jù)平臺(tái)。方法:結(jié)合我國a SAH的診療現(xiàn)狀和未來需求,參考國外臨床數(shù)據(jù)庫建設(shè)的經(jīng)驗(yàn)和理論并結(jié)合a SAH臨床指南及相關(guān)指南,制定科學(xué)、合理的a SAH數(shù)據(jù)庫內(nèi)容及結(jié)構(gòu)。以a SAH的臨床診療數(shù)據(jù)作為目標(biāo)數(shù)據(jù),設(shè)計(jì)數(shù)據(jù)的采集和存儲(chǔ)方式,建立基于WEB的電子數(shù)據(jù)庫錄入、檢索與統(tǒng)計(jì)的醫(yī)療專業(yè)軟件系統(tǒng),為臨床診療和科研提供數(shù)據(jù)檢索與計(jì)算平臺(tái)。采用基于B/S的三層設(shè)計(jì)模式,利用ASP.NET和SQL數(shù)據(jù)庫相結(jié)合技術(shù),實(shí)現(xiàn)數(shù)據(jù)的互聯(lián)網(wǎng)共享。通過與醫(yī)院現(xiàn)有的醫(yī)療管理軟件對接,自動(dòng)導(dǎo)入臨床數(shù)據(jù),實(shí)現(xiàn)數(shù)據(jù)的自動(dòng)采集、整理、導(dǎo)出和聯(lián)網(wǎng)共享,并應(yīng)用該臨床數(shù)據(jù)庫進(jìn)行a SAH相關(guān)的臨床診療和科研。結(jié)果:研制了能夠較為全面反映整個(gè)a SAH患者臨床診療過程的數(shù)據(jù)庫內(nèi)容和結(jié)構(gòu),包括個(gè)人基本資料、入院時(shí)情況、a SAH出血特點(diǎn)、夾閉手術(shù)及介入栓塞、圍手術(shù)期處理、出院及隨訪七個(gè)部分,包含了1000多個(gè)字段和它們的層次關(guān)系結(jié)構(gòu)設(shè)計(jì)。研制了基于數(shù)據(jù)庫內(nèi)容結(jié)構(gòu),以瀏覽器/服務(wù)器為數(shù)據(jù)登錄方式,具備安全性驗(yàn)證、數(shù)據(jù)管理、錄入和導(dǎo)出、聯(lián)網(wǎng)共享以及統(tǒng)計(jì)分析等功能的數(shù)據(jù)庫管理系統(tǒng)。該數(shù)據(jù)庫錄入界面友好,能夠與醫(yī)院的醫(yī)療管理軟件對接,同時(shí)數(shù)據(jù)可以存儲(chǔ)在各種設(shè)備上,從數(shù)據(jù)中心最大的服務(wù)器一直到桌面計(jì)算機(jī)和移動(dòng)設(shè)備,它都可以控制數(shù)據(jù)而不受存儲(chǔ)的位置限制。通過對數(shù)據(jù)庫的應(yīng)用分析,驗(yàn)證了本數(shù)據(jù)庫具備全面、高效的數(shù)據(jù)資料收集能力,以及對數(shù)據(jù)進(jìn)行科學(xué)、有效管理和分析的功能,并對所面臨的困難與問題做進(jìn)一步地調(diào)整。結(jié)論:數(shù)據(jù)庫的研發(fā)使我們擺脫以往臨床研究時(shí)以翻查紙質(zhì)病歷資料為主的傳統(tǒng)模式,將a SAH臨床資料和數(shù)據(jù)信息有序存儲(chǔ),統(tǒng)一管理,并設(shè)計(jì)創(chuàng)新的檢索和計(jì)算模塊,方便、高效地獲取目標(biāo)臨床數(shù)據(jù)及進(jìn)行相應(yīng)統(tǒng)計(jì)分析,同時(shí)借助互聯(lián)網(wǎng)為臨床科研提供隨時(shí)隨地可供調(diào)閱的數(shù)據(jù)平臺(tái)。因此建立一個(gè)適合于我國a SAH診治現(xiàn)狀和發(fā)展要求的數(shù)據(jù)庫是可行的,所建立的該數(shù)據(jù)庫可適用于臨床,能夠基本滿足a SAH臨床診療和科學(xué)研究的需要。
[Abstract]:Objective: aneurysm subarachnoid hemorrhage (SAH) is a common acute cerebrovascular disease with an annual incidence of 635. 3 / 10 million. It is second only to cerebral thrombosis and hypertensive intracerebral hemorrhage and has a high mortality and disability rate. At present, a large number of clinical data have been produced in the study of clinical characteristics, influencing factors, intervention factors, epidemiology and prognosis. These big data need to be integrated, summarized and analyzed. And need a more complete database to input, integrate and extract the required data. From the characteristics of the current level of information in the medical field, any medical science and technology innovation and medical technology development can not be separated from the support of clinical data, the establishment of clinical database is particularly important. Therefore, it is necessary to establish an intelligent clinical database of aneurysm subarachnoid hemorrhage (SAH) to provide a data platform for clinical and scientific research. Methods: according to the current situation and future demand of diagnosis and treatment of aSAH in our country, referring to the experience and theory of clinical database construction in foreign countries, and combining with the clinical guidelines and related guidelines of aSAH, the scientific and reasonable content and structure of aSAH database were worked out. Taking the clinical diagnosis and treatment data of aSAH as the target data, the collection and storage of the data are designed, and the medical professional software system is established based on the electronic database input, retrieval and statistics. To provide a data retrieval and computing platform for clinical diagnosis and research. Using the three-tier design pattern based on B / S and the technology of ASP. Net and SQL database, the data sharing on the Internet is realized. Through docking with the existing medical management software of the hospital, the clinical data are automatically imported, the data are automatically collected, sorted, exported and shared online, and the clinical database is used for clinical diagnosis and scientific research related to the aSAH. Results: the contents and structure of the database which can reflect the whole clinical diagnosis and treatment process of patients with aSAH were developed, including the basic personal data, the characteristics of SAH bleeding at admission, clipping operation and interventional embolization, and perioperative management. Discharge and follow-up seven parts, including more than 1000 fields and their hierarchical structure design. A database management system based on database content structure is developed, which uses browser / server as data login mode and has the functions of security verification, data management, input and export, network sharing and statistical analysis. The database has a friendly interface, docking with hospital medical management software, and data can be stored on a variety of devices, from the largest server in the data center to desktop computers and mobile devices. It can control the data without being restricted by the location of the storage. Through the application and analysis of the database, it is verified that the database has the capability of comprehensive and efficient data collection, as well as the function of scientific, effective management and analysis of the data, and further adjustment is made to the difficulties and problems encountered. Conclusion: the research and development of the database make us get rid of the traditional mode of looking up the paper medical records in the past clinical research, store the clinical data and data information of aSAH in an orderly way, manage the data uniformly, and design the innovative retrieval and calculation module, which is convenient. The target clinical data can be obtained and analyzed efficiently, and the data platform can be accessed anytime and anywhere with the help of the Internet. Therefore, it is feasible to establish a database suitable for the status quo and development requirements of the diagnosis and treatment of aSAH in China. The database can be applied to clinical practice and can basically meet the needs of clinical diagnosis and treatment and scientific research of aSAH.
