【摘要】：課題按照國家衛(wèi)計委關(guān)于加強(qiáng)醫(yī)療機(jī)構(gòu)合理用藥管理和現(xiàn)代醫(yī)院信息化建設(shè)發(fā)展的需要,以現(xiàn)代計算機(jī)技術(shù)、軟件開發(fā)技術(shù)、HIS系統(tǒng)和數(shù)據(jù)庫技術(shù)為基礎(chǔ),構(gòu)建醫(yī)院合理用藥管理信息系統(tǒng)和系統(tǒng)數(shù)據(jù)庫,開發(fā)醫(yī)院合理用藥全程化管控信息系統(tǒng)。本系統(tǒng)通過與HIS系統(tǒng)對接,通過基礎(chǔ)數(shù)據(jù)接口提取HIS系統(tǒng)相關(guān)數(shù)據(jù),在醫(yī)師、護(hù)士、門診以及住院等工作站實現(xiàn)諸多功能。功能主要包括:基礎(chǔ)用藥數(shù)據(jù)與處方查詢、報表管理、處方點評、抗菌藥物臨床應(yīng)用評價、藥歷書寫等。本系統(tǒng)還可以針對不合理用藥危害程度與嚴(yán)重程度,將審查結(jié)果以不同顏色的提示燈的形式告知醫(yī)務(wù)人員,切實實現(xiàn)了對不合理用藥各環(huán)節(jié)、動態(tài)化更新、評價分析與管理決策等功能。為了保證系統(tǒng)的拓展性和適應(yīng)性,滿足不同規(guī)模類型的醫(yī)院和工作模式的實際需求,系統(tǒng)設(shè)計支持不同開發(fā)語言的HIS或者住院患者病歷管理系統(tǒng)的調(diào)用。同時,在國家以“立法”的形式強(qiáng)化抗菌藥物管理的大背景下,本系統(tǒng)按照國家抗菌藥物管理規(guī)范、醫(yī)院規(guī)章制度,以臨床路徑的形式實現(xiàn)了抗菌藥物的信息化管理。該模塊的實現(xiàn),是通過本系統(tǒng)標(biāo)準(zhǔn)數(shù)據(jù)接口,提取HIS系統(tǒng)相關(guān)數(shù)據(jù)或者結(jié)合系統(tǒng)處方審查數(shù)據(jù)庫來實現(xiàn)。數(shù)據(jù)的提取,只局限于規(guī)范抗菌藥物臨床應(yīng)用行為相關(guān)功能的基礎(chǔ)數(shù)據(jù),不涉及HIS系統(tǒng)其他數(shù)據(jù)。醫(yī)師開具抗菌藥物時,系統(tǒng)自動判定醫(yī)師是否存在越權(quán),同時要求醫(yī)師選擇用藥目的、手術(shù)切口類別、微生物送檢等情況,以彈出對話框的形式來提示、規(guī)范醫(yī)師抗菌藥物用藥行為。系統(tǒng)能夠同時滿足醫(yī)師、護(hù)士、藥師以及醫(yī)院領(lǐng)導(dǎo)決策人員不同功能需求,數(shù)據(jù)的統(tǒng)計分析與評價結(jié)果又能有效提高醫(yī)院管理決策的精準(zhǔn)性。課題的研究,對政府節(jié)約寶貴醫(yī)保資金、衛(wèi)生主管部門貫徹落實部門主體責(zé)任、醫(yī)院信息企業(yè)擴(kuò)大市場份額、醫(yī)院提高醫(yī)療質(zhì)量,均具有重大的現(xiàn)實和長遠(yuǎn)意義。
[Abstract]:The project is based on modern computer technology, software development technology, his system and database technology, in accordance with the needs of the State Health and Family Planning Commission on strengthening the management of rational drug use in medical institutions and the development of modern hospital information. The management information system and database of rational drug use in hospital are constructed, and the management and control information system of rational drug use in hospital is developed. Through docking with HIS system and extracting related data of HIS system through basic data interface, this system realizes many functions in doctors, nurses, outpatients and inpatient workstations. The main functions include: basic drug data and prescription query, report management, prescription evaluation, antimicrobial clinical application evaluation, drug calendar writing, etc. The system can also be aimed at the degree of harm and severity of irrational use of drugs, and inform the medical staff of the results of the examination in the form of different color lights, so as to realize the dynamic updating of the various links of irrational use of drugs. Evaluation and analysis and management decisions and other functions. In order to ensure the expansibility and adaptability of the system and to meet the actual needs of different types of hospitals and working modes, the system design supports the HIS of different development languages or the call of inpatient medical records management system. At the same time, under the background of strengthening the management of antimicrobial drugs in the form of "legislation", the system has realized the information management of antimicrobial agents in the form of clinical path according to the national antimicrobial management standard, hospital rules and regulations. The realization of this module is realized by the standard data interface of the system, extracting the relevant data of HIS system or combining with the database of system prescription review. The extraction of data is limited to the basic data of standardizing the clinical behavior related function of antimicrobial drug application, and does not involve other data of HIS system. When doctors prescribe antimicrobial drugs, the system automatically determines whether doctors have exceeded their authority. At the same time, they are required to choose the purpose of medication, the type of surgical incision, the microbiological examination, and so on, so that they can be prompted in the form of pop-up dialog boxes. Standardizing the behavior of doctors in the use of antimicrobial drugs. The system can meet the different functional needs of doctors nurses pharmacists and hospital leaders at the same time. The statistical analysis and evaluation results of the data can effectively improve the accuracy of hospital management decisions. The research of this subject has great realistic and long-term significance for the government to save the valuable medical insurance funds, the health department to carry out the main responsibility of the department, the hospital information enterprise to expand the market share, and the hospital to improve the quality of medical treatment.
