機(jī)動(dòng)車(chē)駕駛員心理人格特征及BDNF基因多態(tài)性與事故傾向性關(guān)聯(lián)研究

【摘要】:目的通過(guò)研究廣西南寧市機(jī)動(dòng)車(chē)駕駛員的心理癥狀、人格特性及5-HT信號(hào)傳導(dǎo)通路上BDNF基因3個(gè)位點(diǎn)多態(tài)性與駕駛員事故傾向性的關(guān)系,發(fā)現(xiàn)影響機(jī)動(dòng)車(chē)駕駛員事故傾向性的危險(xiǎn)因素及易感基因,從心理人格特征及基因水平上研究事故傾向性可能產(chǎn)生的心理-遺傳作用機(jī)制,為今后駕駛員的篩檢進(jìn)而減少交通事故發(fā)生提供科學(xué)依據(jù)。方法本研究采用病例對(duì)照的研究方法,研究對(duì)象均來(lái)自于2016年在廣西工人醫(yī)院體檢中心體檢的機(jī)動(dòng)車(chē)駕駛員,根據(jù)研究對(duì)象的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)確定事故傾向性駕駛員,其中事故組48例,對(duì)照組97例。對(duì)研究對(duì)象收集駕駛員基線資料、相關(guān)心理人格量表及血樣,評(píng)定其心理人格特征,并選取BDNF rs7103411位點(diǎn)、rs11030104位點(diǎn)及rs11030101位點(diǎn)作為候選基因,采用SnapShot技術(shù)分別對(duì)3個(gè)候選基因位點(diǎn)多態(tài)性進(jìn)行基因分型。應(yīng)用Epidata3.1建立數(shù)據(jù)庫(kù)進(jìn)行數(shù)據(jù)整理,運(yùn)用Haploview 4.2進(jìn)行Hardy-Weinberg遺傳平衡性吻合度檢驗(yàn)及單倍體分析,采用SPSS20.0對(duì)數(shù)據(jù)進(jìn)行t檢驗(yàn)、卡方檢驗(yàn)、Logistic回歸分析模型等統(tǒng)計(jì)方法分析。結(jié)果(1)事故組和對(duì)照組機(jī)動(dòng)車(chē)駕駛員的GHQ-12得分比較有統(tǒng)計(jì)學(xué)意義(t=-2.34,P=0.02);(2)事故組未婚駕駛員的抑郁、慌亂、外傾性得分與對(duì)照組相比,差異有統(tǒng)計(jì)學(xué)意義(P0.05);(3)事故組駕齡≤8年駕駛員緊張、憤怒、疲勞、抑郁、慌亂及神經(jīng)質(zhì)因子得分均高于對(duì)照組(P0.05);(4)多因素Logistic回歸模型結(jié)果表明,已婚(OR=0.24,95%CI=0.098~0.65)可能是機(jī)動(dòng)車(chē)駕駛員事故傾向性的保護(hù)因素;(5)事故組機(jī)動(dòng)車(chē)駕駛員神經(jīng)質(zhì)與緊張、疲勞可能有正相關(guān)(緊張r=0.30,疲勞r=0.32,P0.05);(6)BDNF rs7103411、rs11030101及rs11030104三個(gè)位點(diǎn)的基因型及等位基因頻率在事故組與對(duì)照組的比較無(wú)差異(P0.05);(7)采用多因素非條件Logistics回歸分析發(fā)現(xiàn)3個(gè)基因位點(diǎn)的多態(tài)性在校正前后均與機(jī)動(dòng)車(chē)駕駛員事故傾向性的關(guān)系無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);(8)對(duì)照組rs7103411位點(diǎn)中CC基因型組的友善性得分高于CT/TT基因型組,rs11030101位點(diǎn)中AA基因型組的開(kāi)放性得分高于TA/TT基因型組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),事故組rs11030104位點(diǎn)中AA基因型組的友善性得分高于AG/GG基因型組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);(9)BDNF基因單體型在事故傾向性駕駛員事故組和對(duì)照組中的分布不具有統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論機(jī)動(dòng)車(chē)駕駛員心理人格特征可能與事故傾向性無(wú)關(guān)聯(lián);已婚可能是機(jī)動(dòng)車(chē)駕駛員事故傾向性的保護(hù)因素;BDNF rs7103411、rs11030101及rs11030104位點(diǎn)的多態(tài)性可能與機(jī)動(dòng)車(chē)駕駛員事故傾向性無(wú)關(guān),但發(fā)現(xiàn)BDNF基因3個(gè)位點(diǎn)可能與機(jī)動(dòng)車(chē)駕駛員的人格特質(zhì)有關(guān)聯(lián);BDNF基因單體型可能與事故傾向性駕駛員無(wú)關(guān)。
[Abstract]:Objective to study the relationship between the psychological symptoms, personality traits and the polymorphism of BDNF gene in 5-HT signal transduction pathway in Nanning, Guangxi. To find out the risk factors and susceptibility genes that affect the accident tendency of motor vehicle drivers, and to study the possible psycho-genetic mechanism of accident tendency from the psychological personality characteristics and gene level. Methods the case control method was used in this study. The subjects of the study were all motor vehicle drivers who were examined in the physical examination center of Guangxi Workers Hospital on 2016. According to the inclusion criteria and exclusion criteria of the subjects, the accident prone drivers were determined, including 48 cases in the accident group and 97 cases in the control group. The baseline data of the drivers, the relevant psychological personality scale and blood samples were collected from the subjects. The psychological personality characteristics were assessed, and BDNF rs7103411 locus rs11030104 and rs11030101 locus were selected as candidate genes. The polymorphism of three candidate loci was genotyped by SnapShot technique. Epidata3.1 database was used to organize the data. Haploview 4.2 was used to test the genetic balance of Hardy-Weinberg and haploid analysis, and SPSS20.0 was used to test the data. Results the GHQ-12 scores of vehicle drivers in the accident group and the control group were significantly higher than those in the control group. The scores of depression, panic and extroversion of unmarried drivers in the accident group were compared with those in the control group. The difference was statistically significant (P 0.05).
